The well appears to be free flowing without any artificial lift technology present at this time. The well is piped to a gas processing unit and then to three 400 barrel production fluid holding tanks. Production records in RBDMS are current to 2017.
0200834542 2018-07-11 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a four-string casing design with 20 inch diameter surface casing, 13.375 inch diameter intermediate casing, 9.625 inch diameter deep intermediate casing and 5.5 inch diameter production casing with 2.875 inch diameter tubing that appeared to be open flowing. Annular pressure monitoring was in place, the 9.625-inch diameter intermediate casing gauge read 1142 pounds per square inch, the 13.375 inch diameter surface casing gauge read 1283 pounds per square inch. Production equipment included an individual Gas Processing unit. Shared production equipment included a dehydrator, 2 phase separators, contact tower, and 3- 400-barrel production fluid tanks shared with 34013209520000, 34013209560000. The well pad and all equipment were contained in an earthen dike with secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was up to date. There is also a mid-stream valve site on the Southwest Corner of the well pad.

The 13.375 casing was ran and cemented into a 17.5 open hole to a depth of 2261. Halliburton Cementing was on site and cemented the casing with 8710 sacks of Class A Cement. 1839262208 2019-03-20 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well site construction meets ORC 1509.072. The well pad surface and lease road is stone construction and in good repair. Proper well identification is posted at each well head and hazard warning labels are posted on the tank battery. 3 wells are present on the pad. The tank battery has 3 steel 400 barrel tanks situated inside a polymer lined steel containment dike area.
1961191562 2018-07-11 Production Wells No Violations On site inspection accompanied by ODNR inspector Ryan Armstrong. Upon inspection the well was observed to be producing off of a production tree well head assembly with the production valves in the open position. Annular pressure monitoring is present with 1283 pounds per square inch on the 13.375 diameter casing well head section and 1,142 pounds per square inch present on the 9.625 well head section. There is 3 completed wells on the pad with 3 gas processing units present and one contact tower. The produced gas is feeding into a 3rd party mid stream operation on the South West side of the well pad. There is a production fluid storage area on the well pad that consists of a total of 3 steel construction 400 barrel capacity production tanks that are situated inside a polymer lined steel containment dike area. Proper well identification is posted at the well heads and the entry point of the location.
2080124933 2018-04-25 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that was open flowing with valves in the open position. Annular pressure monitoring was in place gauges read 1378 pounds per square inch on the 13.375 inch diameter casing, and 2225 pounds per square inch on the 9.625 inch diameter casing. Production equipment included a Gas Processing unit, line heater, and 3- 400 barrel purge tanks shared with 3401320952, 3401320956. Pressure release lines had been installed on annular casings. Identification tags were present and production was up to date.
