0110238065 2015-07-10 Casing String Witnessed No Violations I received a call from Ron Rudd informing me that Express Energy had lost their auger bit @ 90. They are currently trying to fish it out.
0496189963 2018-07-11 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a four-string casing design with 20 inch diameter surface casing, 13.375 inch diameter intermediate casing, 9.625 inch diameter deep intermediate casing and 5.5 inch diameter production casing with 2.875 inch diameter tubing. The well was currently being treated with nitrogen, down the 5.5 inch diameter casing as an artificial lift clean out process. Annular pressure monitoring was in place, the 9.625-inch diameter intermediate casing gauge read 807 pounds per square inch, the 13.375 inch diameter surface casing gauge read 831 pounds per square inch. Production equipment included an individual Gas Processing unit. Shared production equipment included a dehydrator, 2 phase separators, contact tower, and 3- 400-barrel production fluid tanks shared with 34013209550000, 34013209560000. The well pad and all equipment were contained in an earthen dike with secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was up to date. There is also a mid-stream valve site on the Southwest Corner of the well pad.

1238098875 2018-04-25 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that was open flowing with valves in the open position. Annular pressure monitoring was in place gauges read 967 pounds per square inch on the 13.375 inch diameter casing, and 871 pounds per square inch on the 9.625 inch diameter casing. Production equipment included a Gas Processing unit, line heater, and 3- 400 barrel purge tanks shared with 3401320956, 3401320955. Pressure release lines had been installed on annular casings. Identification tags were present and production was up to date.
1372192476 2015-07-11 Casing String Witnessed No Violations I received a call from Ron Rudd informing me that Express Energy had retrieved their auger bit @ 90. They will resume drilling and cement the conductor pipe.
1514564050 2017-12-13 Preliminary Restoration No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing without any artificial lift technology present at this time. The well is piped to a gas processing unit and then to three 400 barrel production fluid holding tanks. Production records in RBDMS are current to 2017.
1531331068 2018-07-11 Production Wells No Violations On site inspection accompanied by ODNR inspector Ryan Armstrong. Upon inspection the well was observed to be producing off of a production tree well head assembly with the production valves in the open position. There is a third party contractor identified as FSC on site and conducting a Gas Nitrogen treatment to the wellbore by pumping Nitrogen down the well bore 5.5 production casing well had to cause an artificial lift up through and out the production tubing. This is being done to remove built up fluid in the well bore to assist production. Annular pressure monitoring is present with 831 pounds per square inch on the 13.375 diameter casing well head section and 807 pounds per square inch present on the 9.625 well head section. There is 3 completed wells on the pad with 3 gas processing units present and one contact tower. The produced gas is feeding into a 3rd party mid stream operation on the South West side of the well pad. There is a production fluid storage area on the well pad that consists of a total of 3 steel construction 400 barrel capacity production tanks that are situated inside a polymer lined steel containment dike area. Proper well identification is posted at the well heads and the entry point of the location.
1553102939 2019-03-20 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well site construction meets ORC 1509.072. The well pad surface and lease road is stone construction and in good repair. Proper well identification is posted at each well head and hazard warning labels are posted on the tank battery. 3 wells are present on the pad. The tank battery has 3 steel 400 barrel tanks situated inside a polymer lined steel containment dike area.
1560125306 2017-09-07 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing without any artificial lift technology present at this time. The well is piped to a gas processing unit and then to three 400 barrel production fluid holding tanks. Production records in RBDMS are current to 2017.
