0444328369 | 2018-03-28 | Production Wells | No Violations | On site inspection accompanied by ODNR Inspector Andrew Corder. The intention of the inspection is to verify annular pressures. On this day it was observed that the 13.375 well head section had a 2 diameter riser pipe protruding above ground level inside the tin horn cellar, no pressure gauge was present and the valve was in the closed position. The 9.625 diameter well head section also had a 2 diameter riser pipe with a valve in the open position and a pressure gauge present reading 2,780 (psi) pounds per square inch. The company was contacted and a Caleb Sterling was dispatched to location and confirmed which riser pipes were attached to what well head section since the well head lower (B) and complete (A) section is buried in stone inside the tin horn cellar. Mr. Sterling with Rice Energy now (EQT) was directed to install a gauge on the 13.375 well head section riser pipe and verify pressures. Mr. Sterling was also directed to address the pressure on the 2,780 psi on the 9.625 well head section. Mr. Sterling indicated a contract pumper had attempted to release the pressure on the 9.625 well head section on the previous weekend of March 24th- 25th -2018 and they were only successful in blowing the 9.625 well head section pressure down to 1,500 (psi). The area building pressure is the area between the 5.5 diameter casing annuli and the 9.625 casing internal side. Other wells in the direct area owned by the company were also discussed and it was discovered that there is high annular pressure on other wells that the company has been trying to address by blowing the wells down at the well head. It was suggested to Mr. Sterling that the company hire a professional flow back company to safely release the high pressure and begin a monitoring and discovery program to identify and potential well construction defects. |

The well pad does have an earthen berm erected around it. Vegetation appears to be sufficient and no earthen slips or erosion are visible on the day of my inspection however the restoration will have to be followed up on when there is not snow on the ground. There is (2) producing wells on the pad and since my last inspection annular pressure monitoring has been installed on the well head A&B sections with valves and gauges present, the valves are closed and the gauges are frozen over with ice to the point the manual gauges are not readable. There is electronic gauges present also. The riser pipes are tied into flow lines. Attached to the production tree is emergency shut down devices. Production equipment present is (2) gas processing units along with a production facility consisting of (3) steel construction 400 barrel fluid capacity tanks situated inside polymer lined steel containment dike area. Also present on the North side of the well pad is a contact tower and line heater. Proper well identification is posted at the well heads and entry point of the location. |

The drilling rig then installed 397 of 94 pound per foot 20 casing. The final seven feet of pipe was washed to bottom in order to clear the hole of debris. Baker Hughes cemented the casing in place by circulating 125 barrels of class A cement into the annulus of the casing. Sixteen barrels of cement circulated to surface. The wiper plug failed to release from the head until late, resulting in the wiper plug not landing. The casing integrity test will be performed, before drilling the cement plug, in accordance with 1501:9-1-08(D)(3)(b). |
