0893829386 2015-06-28 Drill / Deepen / Reopen No Violations I spoke with the Rice representative on location and he informed me that Falcon Rig #23 was drilling ahead @ 1,800 with the 17.5 open hole and planning to reach total depth in about 440 more feet. The 13.375 casing job should be sometime tomorrow morning.
1277090863 2018-05-24 Production Wells No Violations On site inspection found there is a total of 4 wells in production on the well pad and 3additional wells that are being stimulated. The 4 wells have steel cages over them and concrete barriers around them. Annular pressure monitoring. The tank battery is on the well pad and consists of 6 steel construction 400 barrel capacity tanks situated inside a polymer lined steel containment area. The company has annular pressure transducers monitoring the annular pressure on the 13.375 and 9.625 well head sections.

1533737061 2018-08-08 Production Wells No Violations On site inspection accompanied by ODNR Inspector Ryan Armstrong. Currently there are 4 producing wells on the pad and 3 new wells that the company has service crews on location and completing. The said well is equipped with a lower A section and upper B section and is producing off the 5.5 diameter production casing. The company has installed 2 diameter riser pipes in the cellar, the pipes are connected to the A&B section well head and are equipped with valves and gauges to allow for annular pressure monitoring. The A section which monitors the pressure between the 13.375 and 9.625 diameter casing strings shows 8 pounds per square inch (psi). The B section which monitors the pressure between the 9.625 and 5.5 diameter casing strings shows 1,215 psi on the gauge. The 5.5 diameter production string internal pressure is 1,500 psi and the production flow line pressure is 1,114 psi. There is currently cages placed over the 4 producing wells and barriers around them. The production equipment includes 4 gas processing units and 2 glycol heaters. Upon entering the location on the left side of the well pad is the production fluid facility consisting of 6 steel construction 400 barrel fluid capacity production tanks situated inside a polymer lined steel containment dike area. At the rear of the well pad a midstream operation has been installed. On this day I contacted ODNR Belmont County Engineer Tech Anthony Carson and informed him that the well pad perimeter earthen berm has been removed in the area of the mid stream operation. Mr. Carson will address the issue with the company. I also spoke with the lease operator who manages the well production on site and requested they address the annular pressure on the # 4 well and post proper identification on the # 8 well.
1546353321 2015-06-29 Casing String Witnessed No Violations On this day Flacon Rig # 23 had reached a total depth of 2242. The 13.375 casing was ran and cemented into the 17.5 open hole to a depth of 2211 . Halliburton Cementing was on site and cemented the casing with 925 sks of Class A cement. Cement to surface was established.

1670509272 2018-08-08 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a four-string casing design with 5.5-inch diameter production casing, and 2.875-inch diameter tubing that appeared to be open flowing. Annular pressure monitoring was in place, the 9.625-inch diameter intermediate casing gauge read 1215 pounds per square inch, the 13.375-inch diameter surface casing gauge read 8 pounds per square inch. Production equipment included an individual Gas Processing unit and 6- 400-barrel production fluid tanks shared with 34013209050000, 34013209030000, 34013209020000. The well pad and all equipment were contained in an earthen dike, the dike had been disturbed by midstream on the western edge of the pad. There was secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tag was present, and production was up to date. There were three wells also on location that were in completion process with Mountain State Pressure Systems snubbing rig was currently on the last well to have isolation plugs drilled out.
1677056476 2015-08-12 Preliminary Restoration No Violations On the day of my inspection Patterson Rig # 296 was on location. The site was evaluated to confirm that proper vegetation and erosion controls had been established and maintained. This site has passed the preliminary restoration.
1912522210 2015-06-25 Casing String Witnessed No Violations On this day Falcon Drilling had reached total depth of this section at 318. The 20 casing was ran and cemented into a 24 open hole. Halliburton was on site and pumped 265 sacks of class A cement with cement to surface established. See cement ticket for details.
1967412357 2015-08-27 Drill / Deepen / Reopen No Violations My inspection today finds the drilling rig, Patterson Drilling 296, is drilling a 12.25 hole at a depth of 8430. The company man anticipates reaching a total depth of 8483 within a few hours.
2059604493 2015-09-14 Drill / Deepen / Reopen No Violations My inspection today finds Patterson Drilling 296 is drilling a 8.5 borehole at a depth of 17,266. The azimuth direction of the well is 152 degrees with an inclination of 88.89. The company man notified me that he expects to have a consistent flare though completion of this section.
