0488420566 2018-05-07 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be equipped with a plunger artificial lift technology system. The well is piped to a gas processing unit. From there, it is routed to a production area that contains four 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 50 pounds in between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 0 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing. The production records within RBDMS are currently updated to 2017.
0572485138 2017-10-19 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit and then to a production area that contains four 400-

1436553932 2015-07-08 Casing String Witnessed No Violations On this day Patterson 538 had reached a total depth of 356. The 18.625 casing string was ran and cemented into a 24 open hole. Stingray was on site and cemented the casing with 550 sacks of Class A cement. Cement to surface was established. See cement ticket for details.
1533647818 2018-08-07 Production Wells No Violations Status check inspection found the well pad surface and lease road has a gravel surface and is in good repair. There are 3 wells present on the pad all in production. The said well is equipped with a lower A section and upper B section with 2 diameter riser pipes present at the well head and equipped with valves and manual pressure gauges to monitor annular pressure. The 13.375 x 9.625 diameter casing pressure was zero pounds per square inch (psi) the 9.625 x 5.5 diameter casing pressure was also zero psi. The 5.5 diameter production casing internal working pressure was 300 psi and the production flow line pressure was 350 psi. The well is equipped with production tubing and artificial lift technology. There is a single gas processing unit on location. At the right rear corner of the well pad is the production fluid facility consisting of 4 steel construction 400 barrel capacity production tanks located inside a polymer lined steel containment dike area. There is also a Mark West midstream station at the left front corner entry point of the well pad. Fire extinguishers and emergency shut down control points are present at all the critical areas of the well pad.
1858700198 2015-12-16 Preliminary Restoration No Violations Vegetation has been established at this well site. Erosion and sediment controls are in place and functioning properly. Preliminary Restoration for this site has passed.
1991767336 2015-08-04 Drill / Deepen / Reopen No Violations My inspection today finds the drilling contractor, Patterson Drilling 538, is drilling an 8.5 hole at a depth of 16,702 and an azimuth of 150 degrees. Due to an influx of gas from the well bore the mud weight has been increased to 17.9 pounds per gallon. The Pason system shows 2000 units of gas at the time of inspection. A small flare is burning.
2079010590 2015-03-11 Casing String Witnessed No Violations On site to witness running and cementing of conductor pipe they set 111 of 26 casing and drilled a 32 hole and Stingray cemented it in place with 256 sacks of cement and they had 10 barrel of good cement returns.
572485138 2017-10-19 0:00 PW No Violations 1.00
