0205625572 2015-02-03 Casing String Witnessed No Violations On this day the 13.375 casing was cemented into a 17.5 open hole to a depth of 2051. There was 1600 sks of class a cement pumped with 40bbl of returned to surface. See cement ticket for details. No violations noted.
0256935204 2018-10-23 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing that was equipped with a plunger lift system. At the time of my inspection annular pressure monitoring was in place and the 9.625-inch diameter casing was at 600 pounds per square inch, the 13.375-inch diameter casing pressure was 0 pounds per square inch. Production equipment included an individual Sand Trap, and Gas Processing Unit. As well as 4 400-barrel steel production fluid tanks, and a 50-barrel production fluid tank that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. Identification tags were present, and production was up to date.
0257880299 2015-09-21 Preliminary Restoration No Violations On the day of my inspection completion operations were finalized and the rig down and clean process was underway. The erosion and sedimentation controls were of satisfactory condition and proper vegetation had been established. The preliminary restoration for this well has been completed.
0383880771 2017-07-03 Production Wells No Violations
0524017016 2015-08-25 Completion Testing No Violations Notification was given by the operator that completion operations are aticipated to commence this Friday.
0771879409 2014-12-05 Casing String Witnessed No Violations On the date of inspection I am on site to witness the cementing of the conductor pipe. I was informed by Gulfport Energy consultant, Nick Berelsman, that Rocky Mountain Drilling had drilled a 32 inch diameter hole to a depth of 110 feet. Rocky Mountain then set 110 feet of 26 inch diameter pipe weighing 102.63lbs/ft on bottom. Stingray Cementing LLC broke circulation and pumped 254sk of Class A cement, 15.6ppg, 1.21 yield, 3% CaCl2, and 0.25lbs/sk of celloseal. Stingray displaced cement with 62bbl of fresh water. 22bbl of cement returned to surface and stayed at surface while I was on site.
1041306649 2015-02-09 Drill / Deepen / Reopen No Violations
1146850410 2018-03-16 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains four 400-barrel production fluid holding tanks. There were also three 500-barrrel frac tanks on site. Annular pressures read on the gauges provided appeared to be 500 pounds on between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 150 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing . The production records within RBDMS are currently updated to 2017.
1247298851 2015-02-17 Drill / Deepen / Reopen No Violations Current operations are drilling the 8.5 open hole section @ a depth of 13,803. After total depth is reached the 5.5 casing string will be ran and cemented.
1255772729 2018-03-15 Production Wells No Violations Accompanied Belmont County Inspector Andrew Corder, Reference his Report for further details
1430200111 2015-01-27 Casing String Witnessed No Violations On this day the 18.625 casing was ran and cemented in a 24open hole to a depth of 350. See cement ticket for details. No violations noted.
1815205259 2015-01-26 Drill / Deepen / Reopen No Violations Current operations are preparing to spud. No violations noted.
