0285694161 2018-01-11 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. There are gas processing units on this site. The production area currently contains four 500-barrel production fluid holding tanks. The production records for this well within the RBDMS system are current up to 2017.

The well appears to be free flowing currently with no artificial lift technology in place. The well is piped over to a gas processing unit and then routed to sale and to the tank battery. The tank battery contains four 400 barrel production fluid storage tanks and a 50 barrel fluid storage tank. The production records in RBDMS are current to 2017. I was accompanied by Supervisor Brent Bear during this inspection.
0500818670 2015-08-07 Casing String Witnessed No Violations On site to witness cementing of 18.625 casing Halliburton on site we pumped cement with a total of 1110 sacks to get cement to surface and after cement sat for 6 hours then they bond logged well to comfirm proper cement bond to casing.
1410929759 2018-05-10 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that was open flowing with valves in the open position. Annular pressure monitoring was in place for the 9.625 inch diameter casing gauge read 0 pounds per square inch, the 13.375 inch diameter casing gauge read 50 pounds per square inch. Production equipment included a separator, Gas Processing unit, line heater, and 4- 400 barrel purge tanks shared with 3401320887, 3401320888. Pressure release lines had been installed on annular casings. Identification tags were present and production was up to date.

1725228687 2015-09-01 Preliminary Restoration No Violations On this day Nomac Rig # 82 was beginning to trip into the hole with the 12.25 bit on the 1A well. The site was evaluated to confirm that proper vegetation and erosion controls had been established and maintained. This site has passed the preliminary restoration.
1801430959 2018-09-10 Production Wells No Violations Status check inspection found 3 wells total producing on location. The lease road and well pad surface are of stone surface construction and are in good repair. There is a 3rd party midstream operation along the lease road at the right front corner of the well pad. The production equipment consists of 3 gas processing units and a tank farm that is made up of 4 steel construction 400 barrel production tanks along with a 50 barrel steel tank all located inside a polymer lined steel containment dike area. There is signage posted at the entry point of the lease road and at each well head, hazard warning labels are posted on the tank battery. There is a gate across the lease road that is kept locked when no one is present on site. The well head is equipped with a lower B and upper A section with 2 diameter riser pipes present on the outlet ports on each section that are equipped with valves and gauges. The A section gauge that monitors the pressure between the 13.375 and 9.625 diameter casing strings reads zero pressure and the B section gauge which monitors the pressure between the 5.5 and 9.625 diameter strings reads zero pounds pressure. The 5.5 diameter production casing well head section gauge reads 750 pounds pressure while the tubing head and flow line reads 140 pounds pressure.
