0165670213 2018-01-10 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit and then to a production area that contains six 400-barrel production fluid holding tanks. Production records for this well are current to 2017 within RBDMS.
0438205943 2014-12-09 Casing String Witnessed No Violations On this day the 13.375 casing was ran and cemented in a 17.5 open hole to a depth of 2243. See cement ticket for details. No violations noted.

1310932928 2015-02-17 Drill / Deepen / Reopen No Violations Current operations are drilling ahead in the 12.25 section of the hole @ 8555. After reaching total depth the 9.625 casing will be ran and cemented.
1416235117 2014-11-14 Casing String Witnessed No Violations On the day of my inspection Well Service Group had drilled the 36 open hole and 30 casing was ran and cemented. See cement ticket for details. No violations noted.
1417915954 2014-12-06 Casing String Witnessed No Violations On the date of inspection I am on site to witness the surface casing string cement job. I was informed by Rice Energy consultant, Rich Hild, that the Falcon 23 rig had drilled a 28 inch diameter hole to a depth of 335 feet. Falcon set 24 inch diameter, 79lbs/ft casing to a depth of 311 feet. Baker Hughes pumped water until circulation was established. Baker Hughes pumped 527sk of Class A cement, 15.6ppg, 1.01 yield, 0.1% BA10, 0.35% R3, 0.125% R7C, and 3% SOCL. Baker Hughes displaced cement with 150bbl of fresh water. 5bbl of cement returned to surface and stayed at surface while I was on site.
1477896244 2018-08-21 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a 4-string casing design and was in normal production. Annular pressure monitoring was in place on both the 9.625-inch diameter casing gauge read 1140 pounds per square inch, the 13.375-inch diameter casing gauge read 440 pounds per square inch. Production equipment included an individual Gas Processing unit, and 6- 400-barrel production fluid tanks shared with the other producing wells on the pad. The well pad and all equipment were within earthen containment with secondary containment around the production tanks. There was also a large line heater on location and foam and defoamer tanks near the well head. There were multiple Emergency Shutoff Devices on location, Identification tag were present, and production was current.
1533666838 2018-08-07 Production Wells No Violations Status check inspection found the well pad surface and lease road has a gravel surface and is in good repair. There are 6 wells present on the pad all in production with identification posted at each well. The said well is equipped with a lower A section and upper B section with 2 diameter riser pipes present at the well head and equipped with valves and manual pressure gauges to monitor annular pressure. The 13.375 x 9.625 diameter casing pressure was 717 pounds per square inch (psi) the 9.625 x 5.5 diameter casing pressure was 1,359 psi. The 5.5 diameter production casing internal working pressure was 1,491 psi and the production flow line pressure was 1,089 psi. The well is selling off the 5.5 production casing. There are 4 gas processing units on location along with a large glycol heater. At the left front corner of the well pad is the production fluid facility consisting of 6 steel construction 400 barrel capacity production tanks located inside a polymer lined steel containment dike area. There is also a midstream station at the rear of the well pad. Fire extinguishers and emergency shut down control points are present at all the critical areas of the well pad. The well pad perimeter is protected by an earthen berm.
