0586030763 2015-01-13 Drill / Deepen / Reopen No Violations On the date of inspection I was on site check the status of the conductor that had been set the previous night. I was informed by American Energy Utica drilling superintendent, Vernon Gallo, that the conductor had been cemented by Allied Oil & Gas Services. He informed me that they had seen trace amount of cement reach the surface but it had fell back and would need to be grouted when the next conductor on the well pad was cemented. I witnessed the conductor in the hole with no cement in the cellar and the hole as filled with fluid at approximately 2 feet below the cellar. I spoke with Vernon about proper notification of cement jobs due to the fact that I had been given an hours notice before the job. I was unable to make it because I was halfway into another job.
1013368025 2016-07-19 Drill / Deepen / Reopen No Violations Patterson Rig 284 was working their pipe and drilling in the 8.5 open hole section of the well bore. Once this section is complete and total depth is reached, they will run the 5.5 production casing. The site appeared to be in satisfactory condition.

1490648916 2017-03-27 Preliminary Restoration No Violations Preliminary restoration status check inspection found the well drilling and stimulation process is now complete. The well head has had the completion plugs drilled out and has production tubing installed. The production tree portion of the well head has 2 production flow lines attached with 2 solenoid controlled auto or manual choke valves on the tubing flow cross head. The B section has no flow lines attached and there is annular monitoring gauges installed on all annular well head sections. The well cellars on the 3 existing wells on the pad are free of standing fluid. A pressure testing service is on location pressure testing all production flowlines downstream from the well head. There is 3 gas processing units on location along with line heaters. At the production fluid storage facility a total of 2 steel tanks are present that are capable of holding 400 barrels capacity each. The well pad sits at 700 feet in elevation next to Captina Creek and has an earthen berm present as primary containment with the perimeter drain valves closed. The company man on site indicated they were to begin flow back operations in the next 48 hours and the drain valves for the pad would remain closed during those operations. The location perimeter is fenced by a 8 chain link fence and the location entrance point has signage posted. An emergency shut down device is located at the well pad entrance. The area affected by the well pad construction has erosion control in place and vegetation established.
1975993897 2015-06-03 Casing String Witnessed No Violations On site to witness running and cementing of 18.625 casing they drilled to a depth of 340 of 26 hole and ran 316of casing and O-Tex cemented it in place with 780 sacks of Type H cement and they had 20 bbl of good cement returns.
2068723981 2018-05-02 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains two 500-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 700 pounds in between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 900 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing. The production records within RBDMS are currently updated to 2017.
