0216159646 2018-09-11 Production Wells No Violations On the day of my inspection I witnessed that the well was currently in normal production operations. The well was equipped with 5.5-inch diameter production casing that appeared to be open flowing. Annular pressure monitoring was in place but had no digital readouts available for reference, there were annular pressure bleed down lines on both the 9.625-inch diameter casing, and 13.375-inch diameter casing, at the time of my inspection the 9.625-inch diameter casing valve was closed, and the 13.375-inch diameter casing valve was open and flowing through a choke manifold. Production equipment included a gas processing unit and 2 400-barrel production fluid tanks that were shared with the other wells on the pad. There was secondary containment around the production fluid tanks and gas processing units. Identification tags were present, and production reports were current. Vegetation had been re-established around the location and preliminary restoration has passed according to Ohio Revised Code 1509.72 (A).
0609838455 2015-03-12 Casing String Witnessed No Violations On this day the 13.375 casing was cemented in a 17.5 open hole to a depth of 2400. Nabors ran 1700 sks of class A cement with cement to surface established. See cement ticket for details. No violations noted.
0729094909 2017-06-07 Production Wells No Violations On the day of my inspection the well appeared to be in the production phase. The production line off of the well head lead to a tank battery consisting of two 400 barrel production fluid holding tanks and a gas processing unit. All production mechanics were held within a metal diked area.
1426078811 2015-03-03 Casing String Witnessed No Violations On this day the 24 casing was cemented in a 28 open hole to a depth of 332. Calfrac ran 835 sks of class A cement with cement to surface established. See cement ticket for details. No violations noted.
1426776488 2015-03-04 Casing String Witnessed No Violations On this day the 24 casing was cemented in a 28 open hole to a depth of 332. Nabors ran 835 sks of class A cement with cement to surface established. See cement ticket for details. No violations noted.
1566922421 2019-8-27 0:00: PW No Violations 0.50
2053805977 2018-04-23 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains two 400-barrel production fluid holding tanks. Annular pressures readings supplied by the company were 0 pounds in between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 11 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing. The production records within RBDMS are currently updated to 2017.
