0280480530 2017-08-21 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well site area is fenced in. The well appears to be free flowing without any artificial lift technology present at this time. The well is piped to a gas processing unit and then to a production area and to sales. The production area contains three 400-barrel production fluid holding tanks. There is also a contact tower on site next to the gas processing units. Production records in RBDMS are current to 2017.

0663791847 2014-11-19 Casing String Witnessed No Violations On the date of inspection I was informed by American Energy Utica (AEU) consultant Eugene Lanham that the Alpha Hunter 9 rig had drilled a 28 inch diameter hole to a depth of 360 feet. The rig ran 24 inch diameter 94.7lbs/ft to a depth of 351 feet. The expected mine seam or rubble pile was located between 292 feet and 296 feet. The rig was unable to get the last joint all the way down due to broken down pumps. The job had to wait for the cementors, Allied Oil and Gas Services, to wash pipe down and Allied was held up on another job. Allied pumped fresh water to pipe capacity plus 10bbl to attempt to establish circulation and wash down pipe. Allied established circulation, however, they were unable to wash pipe to bottom. There was 24 feet of stickup leaving the pipe 32 feet below the bottom of the coal/mine seam. This leaves the baskets at 253 feet and 258 feet. AEU decided to cement with the pipe set at 328 feet. Allied pumped 500sk of Class A cement, 15.6ppg, 1.2 yield, 2% Calcium, and 0.25lbs/sk of flake. Allied displaced cement with 181.5bbl of fresh water. 6bbl of cement returned to surface and stayed at surface while I was on site.

The well is piped to a gas processing unit. From there, it is routed to a production area that contains three 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 0 pounds in between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 250 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing. The production records within RBDMS are currently updated to 2017.
1182715749 2015-02-03 Casing String Witnessed No Violations Current operations are rigging up the 17.5 bottom hole assembly to begin drillling the open hole. After reaching the total depth of this section the 13.375 casing will be ran and cemented. No violations noted.
1414969932 2014-11-02 Casing String Witnessed No Violations On the date of inspection I was informed by American Energy Utica consultant Jerry Bright that Express Energy Services had drilled a 36 inch diameter hole to a depth of 100 feet. Express ran 181.0lbs/ft 30 inch diameter casing to bottom. Express pumped 30bbl of water ahead then 275sk of Class A cement, 15.6ppg, 1.18 yield, 2% CaCl, and 0.50lbs/sk of flake. Express displaced cement with 74.8bbl of fresh water. 5 bbl of cement returned to surface but fell back approximately 1 foot. The hole will be topped off to seal the cellar.

1753782059 2018-07-31 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a 3-string casing design with 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing. Annular pressure monitoring was in place, the 9.625-inch diameter intermediate casing gauge read 30 pounds per square inch, the 13.375-inch diameter surface casing gauge read 0 pounds per square inch. There were 3 defoamer tank vaults near the well heads. Production equipment included a Sand Trap, and gas processing unit, there was also a dehydration system with 2 phase separator and glycol injection on location. There are 3- 400 barrel production fluid tanks shared with 34013208620000, 34013208610000, 34013208630000, 34013208760000. There were multiple Emergency Shut Down devices on location, Identification tags were present, and production was up to date. No violations were found at the time of my inspection.
