0611236263 2018-09-25 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing with a plunger lift system. Annular pressure monitoring was in place the 9.625-inch diameter intermediate casing gauge read 1140 pounds per square inch, the 13.375-inch diameter surface casing gauge read 295 pounds per square inch. Production equipment included a sand trap, Line Heater, Gas Processing Unit. There were also 5- 500-barrel steel production fluid tanks inside secondary containment, that were shared with the rest of the producing wells on the pad. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was current in RBDMS.

1410163336 2019-02-07 Preliminary Restoration No Violations Preliminary Restoration found the well site meets ORC 1509.072 with vegetation established and erosion control in place. The well pad surface and lease road is stone construction and in good repair. The lease road is gated and the well pad has a perimeter earthen berm with a 8 chain link fence on the berm. Signage is posted at each of the 8 wells on the pad. All 8 wells have artificial lift production equipment installed on the production tree. Annular pressure monitoring is in place on the well head A&B sections. The production facility consists of 5 steel tanks capable of holding 500 barrels fluid volume each, the tanks are situated inside a polymer lined steel containment dike area. 8 gas processing units are present on the well pad in 2 different locations.
1547383281 2017-10-26 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit and then to a production area that contains five 400-barrel production fluid holding tanks. Production records for this well are current to 2017 within RBDMS.
1949993841 2014-10-25 Casing String Witnessed No Violations On the day of my inspection the 18.625 casing was ran and cemented. See casing ticket for details. No violations noted.
1965488107 2014-10-23 Casing String Witnessed No Violations On the day of my inspection the 24 mine string was set ran and cemented. See cement ticket for details. No violations noted.
1971981335 2014-10-27 Casing String Witnessed No Violations Alpha Hunter rig drilled a 17.5x2110 open hole and set 13.325x2097 of casing. Previous casing set was 18.625x 440. Casing was centralized and no baskets were set. Allied cemented the casing back to surface using class A cement (15.6 ppg; 1.2 yield). 11 bbl of cement returned to surface.
2097688136 2014-10-27 Casing String Witnessed No Violations On the day of my inspection the 13.375 casing was being ran and cemented. See cement ticket for details. No violations noted.
