0449849981 2018-02-13 Drill / Deepen / Reopen No Violations On the day of my inspection Appalachian Rig 1 drilling in the 24-inch diameter section of the well bore at a depth of 294 feet. A open void is expected to be encountered in the section of the hole. Once the open hole is drilled the 18.625-inch diameter casing string will be ran and cemented by C&J Cementing Services.
0450531162 2018-07-18 Completion Testing No Violations Status check inspection on the well completion process found Keane pressure pumping and wireline service on location preparing to begin the well stimulation process. The 5.5 diameter production casing has been pressure tested to 11,600 pounds per square inch and the casing toe sleeve has been opened. 42 total stages are proposed to be stimulated. Annular pressure mechanical gauges are present on the 13.375 and 18.625 casing well head sections. The 9.625 diameter casing well head section has a pressure relief flow line and electronic transducer present.
0740363606 2018-10-16 Completion Testing No Violations On the day of my inspection I witnessed that the well was equipped with a 15,000 pound per square inch manual master valve was on the well head which was shut at the time of my inspection awaiting SIMOPS on producing wells on the pad. XTO Production Superintendent informed me that there was 5000 pounds per square inch on the 5.5-inch diameter casing. Annular pressures were being monitored with gauges and at the time of my inspection the 9.625-inch diameter casing gauge read 0 pounds per square inch and the 13.375-inch diameter casing press was at 700 pounds per square inch. Production superintendent Both the 9.625-inch diameter casing and 13.375-inch diameter casing were tied to a pressure bleed down line and the 13.375-inch diameter casing was open at the time of my inspection. Production Equipment included a gas processing unit and 6- 400-barrel steel production fluid tanks. There was a Key Energy Services coil tubing rig and pressure pumping truck on location at the time of my inspection. After speaking with XTO Production superintendent Tracy Vanscoy he informed me that Key Coil Tubing Rigs blow out preventer malfunctioned, and they were currently waiting on a new Blow Out Preventer which is scheduled to arrive on Friday 10/19/2018.

1531948791 2018-07-18 Completion Testing No Violations On the day of my inspection accompanied by ODNR inspector Bill Stolarik I witnessed that the well was equipped with a four-string casing design and was currently being pressure tested. Keane Pressure pumping, and wireline service was currently on location and had not began completion operations. Both toe sleeves had been open at the time of my inspection. The West Unit 7H was also rigged up for completion operations but would not commence until the West Unit 5H was completed. The producing wells on the pad had VR plugs installed at the well head. All wells on the pad had annular pressure monitoring in place, at the time of my inspection the 9.625 inch diameter casing gauge was 0 pounds per square inch, the 13.375 inch diameter gauge was at 325 pounds per square inch and 18.625 inch diameter casing gauge read 40 pounds per square inch. Keane was preparing to run their perforation tool into the first stage of the well.
1532377769 2018-07-23 Completion Testing No Violations On the day of my inspection I witnessed that Keane Pressure Pumping and wireline service was currently on location performing completion operations. Keane was currently treating the fourth stage of the well out of 42 total stages. Keane Pressure Pumping performed the stimulation with 12,061 barrels of slurry, 433,742 pounds of sand, and pump pressures ranged between 10,000 and 11,000 pounds per square inch. Annular pressure monitoring was in place on all wells on the pad. The 18.625-inch diameter casing was at 250 pounds per square inch and had been shut in since 7/20/2018, when we released pressure on the 18.625-inch diameter there was no fluid that came to surface. The 13.375-inch diameter casing was 200 pounds per square inch and the 9.625-inch diameter casing was 5 pounds per square inch. No violations were witnessed at the time of my inspection.
