0029252134 2015-01-26 Casing String Witnessed No Violations On this day the 18.625 casing was still being ran and cemented in a 24open hole to a depth of 350. The H2S alarm has sounded twice. Safety by design checked the area and confirmed that the alarm did sense H2S and was not a false reading. They loaded the hole and continued operations. No violations noted.
0166655428 2018-06-13 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that was open flowing. Annular pressure monitoring was in place for the intermediate casing gauge read 1073 pounds per square inch, the surface casing gauge read 426 pounds per square inch. Pressure on the surface casing had been recently bled down, there were signs of what appeared to be oil base mud in the cellar. Production equipment included an individual Gas Processing unit. Shared production equipment included a dehydrator, 2 phase separator, contact tower, and 3- 400 barrel purge tanks shared with 34013208450000. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.

The well is piped to a gas processing unit. From there, it is routed to a production area that contains three 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 1946 pounds in between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 1934 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing. The production records within RBDMS are currently updated to 2017.

1204047037 2014-11-07 Casing String Witnessed No Violations On the date of inspection I was informed by Rice Energy consultant Jimmy Groves that the Well Service Group #1 rig had drilled a 28 inch diameter hole to a depth of 340 feet. 24 inch diameter 63.4lbs/ft casing was set at a depth of 320 feet. Baker Hughes established circulation and pumped 494sk of Class A cement, 15.6ppg, 1.2 yield, and 2% CaCl. Baker Hughes displaced cement with 150bbl of fresh water, 25bbl of cement returned to surface and stayed at surface while I was on site.
