0139453288 2018-04-25 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains five 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 1 pounds in between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 850 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing. The production records within RBDMS are currently updated to 2017.
0887083580 2014-11-06 Drill / Deepen / Reopen No Violations On the day of my inspection the rig was drilling ahead with the 17.5 bit. They will run and set the 13.375 casing sometime tomorrow. No violations noted.

1699324191 2018-08-21 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a 4-string casing design and was in normal production. Annular pressure monitoring was in place on both the 9.625-inch diameter casing gauge read 5 pounds per square inch, the 13.375-inch diameter casing gauge read 420 pounds per square inch. Production equipment included an individual Gas Processing unit, and 5- 400-barrel production fluid tanks shared with the other producing wells on the pad. The well pad and all equipment were within earthen containment with secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tag were present, and production was current.
1826640858 2015-08-18 Preliminary Restoration No Violations On this day the wells were awaiting completion operations. The site was evaluated to confirm that proper vegetation and erosion controls had been established and maintained. This site has passed the preliminary restoration.
1933932430 2018-12-05 Production Wells No Violations Status check inspection found well pad surface and lease road of stone construction and in good repair. The well pad site does have an earthen berm that appears to be sufficient and preforming with vegetation established. No movement of the outward slopes was observed. There lease road is equipped with a gale and signage is present at the entry point of the lease road along with being posted on each well head. There is a 3rd party midstream point on the left rear corner of the well pad that consists of a contact tower and glycol heater. There are 5 wells on the pad with 5 processing units. All 5 wells were equipped with riser pipes and transducers on the A&B sections for annular pressure monitoring. The production tree valves are open. The production fluid facility consists of 5 steel construction 400 barrel fluid capacity tanks situated inside a polymer lined steel containment dike area. Vegetation and erosion was observed on all outward facing slopes and all earthen stock pile areas.
