1225695600 2014-08-15 Administrative Inspection Violations Noted Rice Energy (RICE DRILLING D LLC) has neglected to provide proper notification for the commencement of drilling operations. Furthermore, said operator was negligent in providing proper notification of the running and cementing of the conductor casing. Rice Energy shall follow the requirements provided in the Ohio Revised Code.
1413409915 2014-10-15 Casing String Witnessed No Violations Well Service Group Rig 1 drilled a 26 open hole to 340. 320 of 24 (79#/ft) casing was set. 310 of casing from ground level. Previous casing set was 100 of 30 conductor. Baker-Hughes cemented surface casing back to surface using 320 sacks of Class A (15.6 ppg; 1.21 yield). 40% excess cement ran in open hole. Cement additives are 3% CaCl and .25#/sk celloflake. Cement was displaced using 159 bbl of fresh water. 5 bbl of cement returned to surface. 15 of cement left in casing.
1413612505 2014-10-18 Casing String Witnessed No Violations Well Service Group Rig 1 drilled a 22 open hole to 811. 789 of 18 5/8 (54.5#/ft) casing was set. 779 of casing from ground level. Previous casing set was 310 of 24 conductor. Baker-Hughes was onsite to cement mine string back to surface using 700 sacks of Class A (15.6 ppg; 1.20 yield). Rig was having issues with transfer pumped which delayed circulation attempt. Upon trying to circulate the hole, it was discovered that the guide shoe was plugged up. Company man put in a call to Express energy services to come out and perforate casing. Cement job did not occur while onsite.
1413665075 2014-10-18 Casing String Witnessed No Violations Upon arriving onsite, Express Energy Services had already perforated the clogged casing at 785 with 3 holes and 784 with 3 holes. Baker-hughes was rigged up and pumping water to achieve circulation. Circulation was achieved and job was pumped normally. See previous inspection report for hole data. Cement was displaced using 235 bbl of fresh water. 21 bbl of cement returned to surface. 20 of cement left in casing. Casing will be pressure tested and FIT will also be performed.
