0070213886 2017-12-19 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing without any artificial lift technology present at this time. The well is piped to a gas processing unit and then to the production area. The production area contains four 400-barrel production fluid storage tank. The production records within the RBDMS system are current to 2017.
0312464363 2014-09-27 Plug / Plug Back No Violations Paterson rig #253 had lost hole due to planting the 12.25air hammer bit and every thing above it to the 5heavy wall drill pipe decided to plug after several attempts to fish tools out of the hole. The spear was used and lost down the hole on the fish. There was 2.375tubing at 700 with mule shoe at end. There was change over to 5drill pipe. They ran drill pipe above the 700 tail pipe. They tagged the fish at 1864. They used CalFrac as the cementing contractors. They set a 500 plug from 1864-1364. They then pulled up 700 and circulated all KCL Water out of well. They then pulled up to 500 and set the 500-0 plug. Good returns.

1410986750 2014-09-17 Casing String Witnessed No Violations Onsite to witness the cementing of the surface casing. Previous casing set was the 26 conductor to 105. Patterson 253 drilled a 24 open hole to a depth of 340 and set 326 of 18 5/8 (87.5#/ft) casing with 3 centralizers. Calfrac well services cemented the casing in place using 561 sks of Class A (15.6; 1.2 yield) cement at 30% open hole excess. 99 bbl of water used to displace cement. 41 bbl of cement were returned to surface. Job was scheduled for 4 pm. Rig had trouble landing the casing and had to wash out the hole. Casing was set around 10pm. Cementing started at 12 am and pump truck went down and had to be swapped out. Pump job finished around 2am.
1427441507 2015-03-26 Plug / Plug Back No Violations I met with company man on rig and Cal Frac. They had to drill 12 feet of cement out when I arrived on location. They would pull one stand out of drill pipe and then test the treating iron to 3000 psi. Then Cal Frac got nippled up to the drill pipe and they broke circulation. Then the pipe rams were closed on the Blow Out Prevention stack. Cal Frac pressured up the well bore to 500 psi on surface. From 1800 with the hydrostatic weight of fluid the down hole pressure was 1300 psi. They company man and ODNR agreed that that was enough pressure to be placed on the micro channel. There does not need to be a formation break down. Andrew Corder will witness the plugging portion of the well after the mechanical bridge plug is set.
1594832029 2015-01-20 Preliminary Restoration No Violations The preliminary restoration for this well is completed. The pad is in satisfactory condition with proper erosion and sedimentation controls in place. Company shall continue to monitor the location to ensure the pad remains in satisfactory condition. Preliminary restoration passed.
