0643478070 2018-06-19 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that was open flowing. Annular pressure monitoring was in place, the intermediate casing gauge read 0 pounds per square inch, the surface casing gauge read 525 pounds per square inch. Production equipment included a Sand Separator, Gas Processing unit, 4- 400 barrel purge tanks, and a 50 barrel produced water tank shared with 34013208230000, 34013208250000, 34013208850000. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.

1054868635 2015-01-07 Casing String Witnessed No Violations On the date of inspection I am on site to witness the surface casing cemented into place. I was informed by Gulfport consulstant, Matt Knight, that the Patterson 253 rig had spudded in on 1-5-15 and drilled a 24 inch diameter hole to a depth of 525 feet below the Rotary Kelly Bushing (22.5 feet above ground). Franks Casing set 18.625 inch, 87.50lbs/ft casing to a depth of 510 feet below RKB. Cal Frac pumped 758.5sk of Class A cement, 15.6ppg, 1.21 yield, 3% CaCl2, 0.25lbs/sk Celloflake, and 0.17lbs/sk of DCP-DF7 Defoamer. Cal Frac displaced cement with 143.3bbl of fresh water, bumped plug, floats held, 53bbl of cement returned to surface.

Open hole section should reach total depth by tomorrow. They will then rig up to run and cement the 9.625 casing string. They will utilize a DV tool to complete a two stage cement job on this casing string. The area that was effected by the dispersal has been remediated to satisfactory standards. No violations noted.

The well appears to be free flowing without any artificial lift technology present at this time. The well is piped to a gas processing unit and then to the production area. The production area contains four 400-barrel production fluid storage tank. The production records within the RBDMS system are current to 2017.
1320939202 2015-01-12 Casing String Witnessed No Violations On the date of inspection I am on site to witness the cementing of the first intermediate casing. I was informed by Gulfport consultant, Matt Knight, that the Patterson 253 rig had drilled a 17.5 inch hole to a depth of 1973 feet. Franks Casing set 13.375 inch, 54.5lbs/ft casing at 1972 feet. The Patterson 253 rig circulated on the hole prior to rigging up Cal Frac. Cal Frac pumped a lead of 810sk of Class A cement, 15.6ppg, 1.18 yield, 0.25lbs/sk of flake, 0.17lbs/sk of DCP-DF7 Defoamer. Cal Frac then pumped a tail of 789sk of Class A cement, 15.6ppg, 1.19 yield, 1% CaCl2, 0.25lbs/sk of Celloflake, and 0.17lbs/sk of DCP-DF7 Defoamer. Cal Frac displaced cement with 326bbl of water. 61bbl of cement returned to surface.
1427573998 2015-01-16 Drill / Deepen / Reopen No Violations I received a call from Willard Champange regarding a spill on this pad. The gas separator had plugged of causing fluid to mist out of the flare stack. The amount dispersed was .5 bbl of KCL mud. Company commenced clean up following the event. I will check on the clean up and update the system with my findings.
