The well appears to be free flowing without any artificial lift technology present at this time. 1229861254 2019-04-08 Production Wells No Violations Status check inspection found 2 wells on location that have been drilled completed and produced. They are identified as the #2-H and #4-H. These well are temporally shut in with the production flow lines disconnected during the drilling and completion process of 3 additional wells on the pad identified as the # 1-#3 &#5 wells. There is a Rice Energy Mid stream operation on the pad that is currently idle. Also present at the rear of the pad is a above ground storage tank (AST) that is being used for water to stimulate the Razin Kane well pad that sits in close proximity to this pad.

1390739083 2018-08-22 Production Wells No Violations On the day of my inspection I witnessed that the well was no longer in production. Portions of the well head had been removed and the well was no longer connected to production equipment. EQT Drilling Engineer Steve Bishop informed me that they had installed a back-pressure valve in the tubing hanger and Valve Removal plugs in the well head on 8/3/2018. The well heads were contained within a steel cage. Annular pressure monitoring was still in place and the 9.625-inch diameter casing gauge read 1640 pounds per square inch, the 13.375-inch diameter casing gauge read 1630 pounds per square inch. Some of the mobile fluid storage tanks had been moved onto location and EQT was currently waiting on the plastic liners for secondary containment before moving more equipment onto location.

1414679331 2014-10-28 Casing String Witnessed No Violations On the day of my inspection Falcon 39 had drilled the 17.5 open hole and Baker was to cement the 13.375 casing. The cement locked up and cement returns was unable to be established to surface. The casing will be perforated and cement will be squeezed to cover top of cement to surface. No violations noted.
1515949512 2018-06-13 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that was open flowing. Annular pressure monitoring was in place for the intermediate casing gauge read 1481 pounds per square inch, the surface casing gauge read 1472 pounds per square inch. Production equipment included an individual Gas Processing unit. Shared production equipment included a dehydrator, 2 phase separator, contact tower, and 3- 400 barrel purge tanks shared with 34013208170000. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.
1549918947 2018-02-08 Production Wells No Violations On the day of my inspection I witnessed the well was currently shut in and had undergone simultaneous operations preventions. At the time of my inspection I witnessed that all valves were shut and had lock out tag out preventions at the well head. There were transducers being used to monitor annular pressures on the 13.375-inch diameter intermediate casing, at the time of my inspection the digital readout on the gauge was blank, the 9.625-inch diameter deep intermediate casing was tied into the flow line for the well with the valves at the well head and on production equipment in the closed position. EQT completions engineer Andrew Sorg informed me that the producing wells were being monitored by the EQT production operations department, and the 9.625-inch diameter casing was connected to the flow line because the lateral of 34013211890000 was being stimulated and ran adjacent to this well. I informed Carlos Leal the onsite company representative that there needed to be a gauge on the pressure release line so that pressure could be closely monitored.
