0107631637 2018-08-07 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a four-string casing design, 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing with no artificial lift technology. Annular pressure monitoring was in place on both the 13.325-inch diameter surface casing and the 9.625-inch diameter deep intermediate casing, pressures were monitored with transducers with no ability to read pressures at the well head. Both casings had bleed down lines ran to the production fluid tanks, at the time of my inspection the 13.375-inch diameter casing was open flowing into tanks through a choke manifold. There were also two chemical vault tanks near the well heads with foaming and anti-scaling agents. Production equipment included a Gas Processing Unit, that was inside of containment. There were also 3- 400-barrel production fluid tanks inside containment. Production fluid tanks were all shared with 34013208120000, 34013208130000. There were also 3 400-barrel production fluid tanks that were on location and not hooked up to the wells. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.
0204445088 2017-06-07 Production Wells No Violations On the day of my inspection the well appeared to be in the production phase. The production line off of the wellhead lead to a tank battery consisting of five 400 barrel production fluid holding tanks and a gas processing unit. All production mechanics within the tank battery were held within a metal diked area.
1024960536 2014-09-04 Casing String Witnessed No Violations First Class Energy drilled to a depth of 628 and they encountered mine shaft at 574-578 and they ran 620 of 18.625 casing with a basket at 534 and Baker Hughes cemented casing in place with 658 sacks of cement.

Note: Basket at 534 and cement stayed up good and they had two barrel of good cement returns.

1123921627 2014-09-04 Casing String Witnessed No Violations Accompanied by Inspector Dave Ball. Purpose of this inspection was to witness the cementing of a mine string back to surface. Previously cemented casing string is 24x420 inside of 30x100 conductor. First class energy rig 6 drilled a 22x628 open hole. A mine void encountered between 574-578. The rig set 18 5/8x620 (87.5 #/ft) casing with 4 centralizers and 1 collar. Cement basket placed at 534. Baker-Hughes is onsite with 71 sks to perform a balance job below the mine and then cement the annulas of the casing back to surface from the basket with 587 sks. 2 bbl of cement was returned to surface and then job stopped to avoid filling the cellar.
1406674415 2014-07-29 Casing String Witnessed No Violations On the date of inspection I was informed by XTO Company Man Paul Hahn that Express Energy Services has drilled a 36 inch diameter hole to 97 feet. 98 feet of 30 inch diameter conductor casing at 118.64 lbs./ft. was set at 97 feet below surface. I witnessed Express Energy Services (Express) establish circulation in hole prior to pumping cement. Express pumped 57.8bbl (275sk) of Class A cement with 0.50 lbs./sk of flake, 2% CaCl, a weight of 15.6 lbs./gallon and 1.18 yield. 20bbl of cement returned to surface. Cement stayed at surface while I was on site.
