0075212208 2018-08-07 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a four-string casing design, 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing with no artificial lift technology. Annular pressure monitoring was in place on both the 13.325-inch diameter surface casing and the 9.625-inch diameter deep intermediate casing, pressures were monitored with transducers with no ability to read pressures at the well head. Both casings had bleed down lines ran to the production fluid tanks, at the time of my inspection the 13.375-inch diameter casing was open flowing into tanks through a choke manifold. There were also two chemical vault tanks near the well heads with foaming and anti-scaling agents. Production equipment included a Gas Processing Unit, that was inside of containment. There were also 3- 400-barrel production fluid tanks inside containment. Production fluid tanks were all shared with 34013208140000, 34013208130000. There were also 3 400-barrel production fluid tanks that were on location and not hooked up to the wells. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.

0434944322 2014-09-07 Casing String Witnessed No Violations Purpose of this inspection was to witness the cementing of the surface casing back to surface. First Class Energy Rig 6 drilled a 28x421 open hole. 24x401 (95#/ft) casing set. Casing was centralized with 4 centralizers. Previous casing set was a 30 conductor to 95. Baker-Hughes is onsite with 605 sks of class A cement (15.6 ppg; 1.21 yield). Cement additives are 3% CaCl and .25#/sk celloflake. 30% excess ran in the open hole. 189 bbl of fresh water used for diplacement. 20.5 bbl of cement returned to surface.

The production line off of the wellhead lead to a tank battery consisting of five 400 barrel production fluid holding tanks and a gas processing unit. All production mechanics within the tank battery were held within a metal diked area.

Previous casing set was 24 to a depth of 401. Baker Hughes is onsite with 656 sks of Class A (15.6; 1.21 yield) to cement the casing back to surface. 50% excess ran in open hole. 30 bbl cement returned to the surface. Rig was still running joints into the hole upon my arrival and I was not updated that the job was running behind schedule.

1901938328 2014-07-28 Casing String Witnessed No Violations On the date of inspection I was informed by XTO Company Man Paul Hahn that Express Energy Services has drilled a 36 inch diameter hole to 101 feet. 30 inch diameter casing at 118.64 lbs/ft was set at 100 feet. I witnessed Express Energy Services (Express) establish circulation in hole prior to pumping cement. Express pumped 57.8bbl of Class A cement with 0.50 lbs/sk of flake, 2% CaCl, a weight of 15.6 lbs/gallon and 1.18 yield. 15bbl cement returned to surface. Cement stayed at surface while I was on site.

2064431787 2015-08-07 Production Wells No Violations On this day I received a call from Chris Holmes with the Ohio EPA at 12:02 pm who indicated they had received a complaint from a John Vigoffi about a gas leak in the area of KEY in Richland Township Belmont County. I responded and found workers were conducting maintained operations on the Summit Midstream Kaldor Dehy station that receives gas from the Albright well pad. The company representative on site had indicated that they were running a pig sweep on the newly conducted 20 sales line and only air was being discharged. The location of the pad is at EPE 25-39.95450 X 80.82932. Due to Jurisdictional boundaries the information was forwarded over to the Ohio EPA by ODNR Emergency Response Director Scott King. My Direct supervisor Brent Bear was also updated on the findings.
