0353420258 2018-08-22 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a 4-string casing design with 5.5-inch diameter production casing that appeared to be open flowing. Annular pressure monitoring was in place, the 9.625-inch diameter intermediate casing gauge read 110 pounds per square inch, the 13.375-inch diameter surface casing gauge read 150 pounds per square inch. Production equipment included a Sand Trap, a line heater, and vertical fluid separator. There are three 400-barrel production fluid tanks shared with the other well on the pad. There were multiple Emergency Shut Down devices on location, Identification tags were present, and production was up to date. No violations were found at the time of my inspection.

The area contains the gas processing units and three 400 barrel production fluid holding tanks. Production records in RBDMS are current to 2017.
0430455857 2018-11-15 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction meets ORC 1509.072 with vegetation established. No erosion was noted on the containment berm or well pad slopes. The lease road and well pad surface was of stone construction and in good repair. The well pad does have an earthen berm around the perimeter with a 8 chain link fence surrounding the well pad. Signage is posted at the lease road entry point and the well pad entry point as well as at each well head. Hazard warning is posted on the tank battery which is situated on the well pad and consists of (3) steel construction 400 barrel fluid volume tanks that are situated inside a polymer lined steel containment dike area. The well head A&B sections are equipped with 2 diameter riser pipes with valves and gauges present for annular pressure monitoring. The A section which is the 13.375 diameter casing pressure measures 200 pounds pressure. The B section which monitors the 9.625 diameter casing pressure measures 100 pounds pressure. There is 500 pounds pressure on the 5.5 diameter production casing. There is a production tree attached to the B section equipped with a emergency shut down device.

0945597943 2014-10-28 Casing String Witnessed No Violations On the day of my inspection the 24mine string was being ran and cemented. See casing ticket for details. No violations noted.
0947355711 2014-10-28 Casing String Witnessed No Violations Nomac 81 drilled a 27x279 open hole and set 24x277 of casing. Operator does not believe a mine void was encountered but a coal seam is believed to be at 231-240. Had to wait for cementers to arrive onsite prior to washing the last joint of casing down. Job was delayed. Baker Hughes pumped 407 sks of class A (15.6 ppg; 1.20 yield) cement on the bottom for a balance job. Cement baskets were set at 216 and 221. Baskets were activated by dumping LCM and gravel down the backside of the casing. 5 bbl of cement was pumped down the backside to set up on basket. Cementers waited an hour for the cement to set up and then pumped 38 bbl with 1 tubing on top of the basket. Tubing was only able to reach 100 feet into the backside. Good cement back to surface.
1020959699 2018-04-06 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains three 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 150 pounds on between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 100 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing. The production records within RBDMS are currently updated to 2017.

The piping from the well leads to a gas processing unit and then to the tank battery. The tank battery consists of three 400 barrel production fluid tanks. The site appears to be in satisfactory condition.

2030991495 2015-10-28 Production Wells No Violations This inspection was conducted as a result of a complaint received on 10/27/2015. Complaint alleged vibration initiated by the well pad is causing damage to the residence. The wells on this site have been drilled and completed. There are three line heaters and three 400 bbl production fluid tanks on site. The wells have all necessary equipment to be considered capable of production and were producing on the day of my inspection. I have contacted Ascent Resources to discuss their findings on this complaint and possible noise mitigation. I will meet with an Ascent representative on 10/29/2015 @ 10am to further discuss this complaint. Please refer to complaint # 5166 concerning this matter. No violations were noted on site.
