0123962338 2016-09-27 Production Wells No Violations This well is equipped properly to be considered capable of production. The piping from the well leads to a gas production unit then to the tank battery. The tank battery consists of four 400 barrel production fluid tanks. This site appeared to be in satisfactory condition.
0530758695 2018-07-05 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing, which appeared to be open flowing. Annular pressure monitoring was in place the 9.625 inch diameter intermediate casing gauge read 0 pounds per square inch, the 13.375 inch diameter surface casing gauge read 125 pounds per square inch. Production equipment included a Sand Trap, Gas Processing Unit. There were also 4- 400 barrel stock tanks and a 50 barrel produced water tank shared with 34013207940000, 34013207930000, 34013207960000, 34013207920000. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.
0858756959 2015-09-10 Preliminary Restoration No Violations On the day of my inspection the well was properly equipped to be considered capable of production. The tank battery consisted of four 400 bbl oil storage tanks. There was a separation area on site. All units were in containment and the containments areas were reasonably free of liquids. The erosion and sedimentation controls were of satisfactory condition and proper vegetation had been established. The preliminary restoration for this well has been completed.

1410202128 2014-09-08 Drill / Deepen / Reopen No Violations Received phone call from company man informing me that there was a spill on the pad. Upon arrival to the pad the company man informed me that they were pressuring washing equipment during the night and in the morning two wet spots were identified outside of the primary containment. Total affected area outside of the primary containment was approximately 140-180 sq. ft. In both spots the company excavated a 6 deep by 1 wide by 10 long ditch to catch any fluid leaking from underneath the containment. The first ditch contained an estimated 10-15 gallons (1432 ppm chlorides per quanatab) and the second contained no more than 10 gallons (1825 ppm chlorides per quanatab). Liner repair guys had identified holes in the liner and were repairing them during this inspection. Rig hand was sucking water from the first ditch by the time I exited the pad. Inspector will follow up on 9-10-14 to verify that remediation has taken place. Current drilling operation is picking up equipment to start drilling the curve. 8 3/4 hole is at a depth of 8180.
