0560923131 2015-05-14 Casing String Witnessed No Violations On the date of inspection I am on site to witness the cementing of the conductor pipe. I was informed by XTO consultant Paul Hahn that Express Energy Services had drilled a 36 inch diameter hole to a depth of 100 feet. Express set 30 inch diameter, 126lbs/ft conductor pipe on bottom. Express pumped 275sk of Class A cement, 15.6ppg, 1.18 yield, 2% CaCl2, and 0.50lbs/sk of flo-seal. Express displaced with 75bbl of fresh water. 20bbl of cement returned to surface and stayed at surface while I was on site.

1275629000 2018-08-01 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a four-string casing design, 5.5-inch diameter production casing and 2.875 inch diameter tubing that appeared to be open flowing with no artificial lift technology. Annular pressure monitoring was in place on both the 13.325-inch diameter surface casing and the 9.625-inch diameter deep intermediate casing, pressures were monitored with transducers with no ability to read pressures at the well head. Both casings had bleed down lines ran to the production fluid tanks, at the time of my inspection the 13.375-inch diameter casing was open flowing into tanks through a choke manifold. Production equipment included a Gas Processing Unit, that was inside of containment. There were also 4- 400-barrel production fluid tanks inside containment. Production fluid tanks were all shared with 34013207850000. A 500 barrel frack tank was on location and no longer hooked up to the wells. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.
1332129054 2019-02-25 Production Wells No Violations Status check inspection found 4 wells total on the well pad with all 4 wells now in production. The said well is equipped with a lower A and upper B section with annular pressure monitoring in place on each well. The well is producing off a production tree. (2) Production facilities are present on the pad with the facility associated with the said well being equipped with (4) steel 400 barrel capacity tanks situated inside a polymer lined steel containment dike area. No perimeter earthen berm is present around the pad. Identification is posted on the well head. The lease road and well pad surface are stone construction and in good repair.

The well is piped to a gas processing unit and then to a production area that contains four 400-barrel production fluid holding tanks. Production records for this well are current to 2017 within RBDMS.
