1337479571 2018-07-10 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a three-string casing design with surface, intermediate and 5.5 inch diameter production casing, with 2.875 inch diameter tubing, the well appeared to be open flowing with no artificial lift technology in place. Annular pressure monitoring was in place, the 9.625-inch diameter intermediate casing gauge read 1105 pounds per square inch, the 13.375 inch diameter surface casing gauge read 14.5 pounds per square inch. Production equipment included an individual Gas Processing unit. Shared production equipment included a dehydrator, 2 phase separators, contact tower, and 3- 400-barrel purge tanks shared with 34013208700000, 34013207780000, 34013210770000, 34013210780000, 34013210790000. The well pad and all equipment were contained in an earthen dike with secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was up to date.
1406770202 2014-07-30 Casing String Witnessed No Violations On site to witness the cementing of the 13.375 first intermediate casing. Falcon 39 drilled a 17.5 x 2094 open hole. Express set 2115 of 13.375 54.5#/ft casing. Baker Hughes is onsite with 1859 sacks of class A (15.6 ppg; 1.2 yield) cement to perform the job. The company is using 50% excess and leaving 40 on cement in the casing. Cement returns were viewed on the surface.

1463754299 2016-05-17 Production Wells No Violations I was on site with Brent Bear to conduct an inspection of this well. The well was equipped adequately to be considered capable of producing. The company was preforming well head maintenance which consisted of greasing the well head.

I performed an on-site evaluation to confirm the proper vegetation was established and erosion and sedimentation controls were maintained. This site has passed the preliminary restoration inspection.
