0443955016 2015-08-13 Preliminary Restoration No Violations On the day of my inspection the well appeared to have all equipment to be considered capable of production. The tank battery contained six 400 barrel production fluid storage tanks. The site was evaluated to confirm that proper vegetation and erosion controls had been established and maintained. This site has passed the preliminary restoration.
0712620973 2018-05-07 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that was open flowing with valves in the open position. Annular pressure monitoring was in place for the 9.625 inch diameter casing and gauge read 0 pounds per square inch. There were no gauges present on the 13.375 inch diameter casing. Production equipment included a Gas Processing unit, line heater, and 6- 400 barrel purge tanks shared with 3401320763, 3401320761. Pressure release lines had been installed on annular casings. Identification tags were present and production was up to date.
1007183104 2017-12-26 Production Wells No Violations This well is equipped with 5.5-inch production casing. The well appears to be free flowing at this time with no artificial lift technology present. The well is piped to a gas processing unit and then to a production area containing six 400 barrel fluid storage tanks.The production records in RBDMS are current to 2017.

1395419207 2014-03-19 Casing String Witnessed No Violations On site to witness cementing of conductor pipe it was noted that operator had run 66 of drive pipe36 and then they ran 76 of 26 casing and cemented it in place with 288 sacks of Class A Cement with good returns and it stayed up good.

Note: Stingray Cementing pumped cement, and spud date on this well was noted@3/14/14.

1577002306 2014-07-28 Completion Testing No Violations On the day of my inspection frac operations had moved off the pad while containment liners were checked and repaired for precautionary measures. No violations noted.

The well was equipped in a manner that made it appear to have the capability of producing. The location included a gas processing unit and a tank battery that consisted of six 400 barrel production tanks.
