0373246073 2014-03-11 Casing String Witnessed No Violations On the day of my inspection the conductor casing was ran and cemented. See cement ticket for details. No violations noted.
0497416798 2017-09-26 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing without any artificial lift technology present at this time. The well is piped to a gas processing unit and then to the production area. The production area contains four 400-barrel production fluid storage tank. The production records within the RBDMS system are current to 2017.
0714711546 2016-09-07 Production Wells No Violations During this inspection I was accompanied by Austin Gutridge. This well is equipped properly to be considered capable of production. The piping from the well leads to a gas production unit then to the tank battery. The tank battery consists of four 400 barrel production fluid tanks. This site appeared to be in satisfactory condition.
1086156146 2018-07-03 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing, and 2.875 diameter tubing with a plunger lift system. Annular pressure monitoring was in place the 9.625 inch diameter intermediate casing gauge read 10 pounds per square inch, the 13.375 inch diameter surface casing gauge read 0 pounds per square inch. Production equipment included a Sand Trap, Gas Processing Unit, 4- 400 barrel stock tanks and a 50 barrel produced water tank shared with 34013207570000, 34013207590000. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.
1359170810 2014-04-28 Casing String Witnessed No Violations On the day of my inspection the 13.375 casing was ran and cemented. See cement ticket for details. No violations noted.
1434570748 2014-08-11 Completion Testing No Violations Onsite with Inspector Andrew Corder. We checked sump area and surrounding puddles for elevated chloride levels. See his report this date for further information.

1546623884 2014-08-11 Completion Testing No Violations On the day of my inspection I tested the sump area for elevated chlorides. I also checked surrounding puddles. I found that there was still a presence of elevated chlorides and I will conduct further investigation later this week with a company representative. We will determine the best remedial action at that time and proceed.
1670424935 2017-12-12 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. There are gas processing units on this site. The production area currently contains four 400-barrel production fluid holding tanks. The RBDMS system is current with production records up to 2017.
1701116646 2014-07-03 Completion Testing No Violations On the day of my inspection completion operations were taking place. All wells are being fraced at the same time. (zipper frac). No violations noted.
1737532672 2014-05-01 Casing String Witnessed No Violations On the day of my inspection the 9.625 first intermediate was ran and cemented. See cement ticket for details. No violations noted.
1924011206 2015-09-10 Preliminary Restoration No Violations On the day of my inspection the well was properly equipped to be considered capable of production. The tank battery consisted of four 400 bbl oil storage tanks. There was a separation area on site. All units were in containment and the containments areas were reasonably free of liquids. The erosion and sedimentation controls were of satisfactory condition and proper vegetation had been established. The preliminary restoration for this well has been completed.
