0410582935 2018-03-01 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains six 400-barrel production fluid holding tanks. The production records within RBDMS are currently updated to 2017.
1084426033 2018-05-30 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that was open flowing with all valves in the open position. Annular pressure monitoring was in place for the intermediate casing gauge read 1350 pounds per square inch, the surface casing gauge read 560 pounds per square inch. Production equipment included a Separator, Gas Processing unit and line heater, production equipment was shared with 34013207420000, 34013207430000. There are 6- 400 barrel purge tanks and a 50 barrel brine storage tank shared with 34013207420000, 34013207430000. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.

1222989392 2018-09-13 Production Wells No Violations Status check inspection found 3 well present and producing on the well pad. Annular pressure monitoring is in place and the wells are selling off the production tubing with no artificial lift system present. There is one gas processing unit present on location. The production fluid storage area is on the right front corner of the well pad and consists of 6 steel tanks capable of holding 400 barrels fluid volume each along with a single 50 barrel steel tank. All tanks are situated inside a polymer lined steel containment dike area There is a third party mid stream operation take point on the left rear corner of the well pad. Signage and hazard warning labels are posted and emergency shut down devices are situated at all critical operation areas on the pad. The well pad surface and lease road is of stone construction and in good repair. There is a gate on the lease road.

2057914500 2015-08-13 Preliminary Restoration No Violations On the day of my inspection the well appeared to have all equipment to be considered capable of production. The tank battery contained 400 barrel production fluid storage tanks. The site was evaluated to confirm that proper vegetation and erosion controls had been established and maintained. This site has passed the preliminary restoration.
