0174500943 2015-08-13 Preliminary Restoration No Violations On the day of my inspection the well appeared to have all equipment to be considered capable of production. The tank battery contained six 400 barrel production fluid storage tanks. The site was evaluated to confirm that proper vegetation and erosion controls had been established and maintained. This site has passed the preliminary restoration.
0783433253 2018-02-06 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well is free flowing as it does not appear to have any artificial lift technology present at this time. The well is piped to a gas processing unit and then to a production area. The production area contains six 400-barrel production fluid holding tanks. The production records within RBDMS are current to 2017.
1120233500 2018-08-01 Production Wells No Violations Status check inspection found the well pad surface and lease road of stone construction, the lease road does have identification posted and a gate present. As you enter the well pad there is a Mark West midstream station on the left side. Situated in the center of the well pad is 3 producing wells all equipped with emergency shut down devices on the production tree outlet side past the 2 primary master valves, the production flow line pressure is 250 pounds per square inch (psi). The 5.5 production casing internal pressure is 400 psi. The well head A &B sections is equipped with valves and riser pipes with gauges present to monitor annular pressure. The A section riser pipe which gauges the pressure between the 13.375 and 9.625 diameter casing strings showed 50 pounds pressure. The B section which is the pressure between the 9.625 and 5.5 diameter casing strings also had 100 pounds pressure showing on the gauge. The wells are selling into gas processing units with a total of 3 units present on location. At the front right hand corner of the well pad is the production facility consisting of 6 steel construction 400 barrel production tanks situated inside a polymer lined steel containment dike area. Fire extinguishers and emergency shut down control points are situated on the pad at all critical areas. Proper well identification is posted at each well along with hazard identification on each production tank. The well pad earthen berm is in good repair and the perimeter area of the well pad has vegetation established.

1348465312 2018-06-12 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that was open flowing. Annular pressure monitoring was in place for the intermediate casing gauge read 300 pounds per square inch, the surface casing gauge read 150 pounds per square inch. Production equipment included a Separator, line heater and Gas Processing unit. Production equipment included 5- 400 barrel purge tanks, a 500 barrel purge tank, and a 50 barrel brine storage tank shared with 34013207040000, 34013206740000. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.
