Cement off methane producing zone and move in H & P 284 to drill out cement load hole with potable water and use thread style casing. No violations noted.
1374849697 2013-07-22 Drill / Deepen / Reopen No Violations I was onsite to witness some of the stages of the hydraulic fracturing of the well. The operation is currently shut down because the have lost tools down hole and were unable to fish them out. They were going to set a packer and continue on but decided not to. They are going to put operations on hold until they can bring in a snub unit in several months and then complete the rest of the stages of the job.
1592563762 2019-02-07 Preliminary Restoration No Violations Preliminary Restoration found the well site meets ORC 1509.072 with vegetation established and erosion control in place. The well pad surface and lease road is stone construction and in good repair. The lease road is gated and the well pad has a perimeter earthen berm with a 8 chain link fence on the berm. Signage is posted at each of the 8 wells on the pad. All 8 wells have artificial lift production equipment installed on the production tree. Annular pressure monitoring is in place on the well head A&B sections. The production facility consists of 5 steel tanks capable of holding 500 barrels fluid volume each, the tanks are situated inside a polymer lined steel containment dike area. 8 gas processing units are present on the well pad in 2 different locations.

1751650366 2013-05-06 Drill / Deepen / Reopen No Violations On the day of my inspection HAD drilling had reached TD and was pulling out of the hole to run 18.625 mine string. No violations noted.
1928770410 2018-09-25 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing with a plunger lift system. Annular pressure monitoring was in place the 9.625-inch diameter intermediate casing gauge read 0 pounds per square inch, the 13.375-inch diameter surface casing gauge read 0 pounds per square inch. Production equipment included a sand trap, Line Heater, Gas Processing Unit. There were also 5- 500-barrel steel production fluid tanks inside secondary containment, that were shared with the rest of the producing wells on the pad. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was current in RBDMS.
