0156240229 2018-05-09 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be equipped with a plunger artificial lift technology system. The well is piped to a gas processing unit. From there, it is routed to a production area that contains one 400-barrel production fluid holding tank. Annular pressures read on the gauges provided appeared to be 0 pounds in between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing.

The containment on location appeared to be of satisfactory condition. Circulation was lost when pumping fresh water. Well started to vacuum and LCM and gel was pumped to heal the formation. Attempts to regain circulation were unsuccessful and casing will be pulled to recondition hole with the drill string. No violations noted.

1511668431 2013-03-12 Administrative Inspection No Violations On the day of my inspection I met with Dean of the Hess Corp. We discussed the existing wells on the pad and how to protect them from any accidents when future drilling commences. It was determined that Hess will be required to put up steel cage barriers with welded tops and cement blocks to protect existing wells. Hess will also be required to reinstall E&S controls. No violations noted.
1512333950 2013-06-23 Casing String Witnessed No Violations On the day of my inspection 18.625 casing was ran and cemented by Halliburton. The job was successful with 31bbl of returns. See casing ticket for more information.
1662205589 2018-12-26 UrbanProduction Wells No Violations Status check inspection found the well equipped with a lower A and upper B section with riser pipes present on both well head sections equipped with transducers to monitor annular pressure. The well is producing through production tubing with a artificial lift system present on the production tree. Both gauges and transducers are present on the production tree and 5.5 diameter production casing well head section. Identification is posted at the well head and the well pad location is protected by an 8 chain link fence with 3 strands of barbed wire at the top. Along the lease road to the pad is a separate adjoining well pad that houses the production facility. There is a 8 chain link fence around this facility also. There is a single 300 barrel fiberglass production tank present on the pad , the tank is situated inside a polymer lined steel containment dike area. There is no earthen berm around the production pad, the well pad does have a berm. Also present is various gas processing equipment. Well identification is posted on the production tank.
