0413429606 | 2011-02-09 | Preliminary Restoration | No Violations | On day of inspection well is supplied with a 9 5/8cas,7cas,4.5cas,1.5tubing.a pump jack .There is a 100bbl tank and dike and id are in place. Preliminary restoration passed. |
0465677740 | 2013-04-23 | Production Wells | No Violations | On the day of my inspection the well was equipped with a pumping unit and capable of production. The tank battery contents included one 100bbl tank, brine storage tank, and a mechanical separator. No violations noted. |
0539214875 | 2018-10-17 | Production Wells | No Violations | On the day of my inspection I witnessed that production equipment included a 100-barrel production fluid tank, 37-barrel plastic brine tank, vertical separator and 2-inch meter run with drip all contained within an earthen dike. The well was equipped with 8.625-inch diameter surface casing, 4.5-inch diameter production casing and 1.5-inch diameter tubing and rods, with a General pumping unit and natural gas motor that was supplied through a drip on the production casing. At the time of my inspection all valves were open, there was an identification tag on the 100-barrel tank and production had been reported as zero since 2014. The Production area and well equipment were all surrounded by vegetation, and there was not a chart in the 2-inch meter run. |
1173308330 | 2017-08-28 | Production Wells | No Violations | This well is equipped with 4.5-inch diameter casing, a pumping unit, tubing, and rods. The pumping unit is connected to the rods. The pumping unit also has a motor and belts. The production area contains one 100-barrel production fluid storage tank, a 25 barrel production fluid storage tank, and a vertical mechanical separator. No production shown in RBDMS since 2014. |
1541604435 | 2018-11-07 | Production Wells | No Violations | Status check inspection found the well equipped with 4.5 diameter production casing and 1.900 production tubing. The is 2 diameter steel flow lines attached to the tubing and casing well head with valves present in line and in the open position. Also present and attached to the surface casing is a gas supply that runs to the pumping unit drive engine. The well is set up to be produced by the means of a pumping unit with a drive engine present however the drive belts are not presently attached. The unit and rods are also stuck in mid stroke indicating the downhole rods or pump is stuck. The production facility sits up along Patterson Hill Road and consists of a single 100 barrel steel storage tank and a 1,200 gallon plastic tank along with a mechanical separator and gas sales meter. The gas meter has no sales chart and however a pressure gauge is present on the separator showing 25 pounds pressure, all valves are open through the separator to the sales meter and production flow line. Production records are current however no commercial production in paying quantities has been reported since 2014. |
1558455218 | 2019-05-21 | Plug / Plug Back | No Violations | A service rig owned by Knox Energy is rigged up on the well pulling the 1.900 production tubing and rods out of the well. A total of 59 joints of tubing and the mud anchor was pulled, the mud anchor was removed and the tubing ran back in the well open ended. The tubing was tallied. ODNR Inspector Ryan Armstrong was on site and will witness the plug job the following day. All calculations were reviewed. |

1933513546 | 2019-05-23 | Plug / Plug Back | No Violations | On the day of my inspection I witnessed Berry Well Service had ran into the well bore and tagged cement at 940 feet. Berry Well Service then ran 1.9-inch diameter tubing to 898 feet, Cementco was on location and pumped 6.3 barrels, 30 sacks of 15.6 pound per gallon Class A cement with a yield of 1.18 cubic feet per sack and displaced to 793 feet with 2 barrels of fresh water. Berry Well Service then tripped out of the hole with the 1.9-inch diameter tubing to 100 feet and pumped 30 sacks of 15.6 pound per gallon Class A cement with a yield of 1.18 cubic feet per sack for the top plug. I spoke with Knox Energy representative Bill Mitten and he informed me that they will wait one day and make sure the cement for the top plug has not dropped and cut the casing 36 inches below ground level. |
2001109591 | 2019-05-22 | Plug / Plug Back | No Violations | On the day of my inspection I witnessed Berry Well Service had ran 2,050 feet of 1.9-inch diameter tubing inside of the 4.5-inch diameter casing. Cementco was on location and pumped 6.3 barrels, 30 sacks of 15.6 pound per gallon Class A cement with a yield of 1.18 cubic feet per sack and displaced to 1,650 feet with 4.1 barrels of fresh water. Berry Well Service then tripped out of the hole and laid down the 1.9-inch diameter tubing Berry Well Service then ran in the hole with a casing cutting tool to 1,320 feet and separated casing. Berry Well Service then pulled 8 joints of 4.5-inch diameter casing to 1,069 feet and pumped 19 barrels, 90 sacks of 15.6 pound per gallon Class A cement with a yield of 1.18 cubic feet per sack and displaced to 769 feet with 12.5 barrels of fresh water. Berry Well Service then tripped out of the hole with the remaining 4.5-inch diameter casing. Knox Energy then attempted to run 1 inch down the backside of the 8.625-inch diameter surface casing to 15 feet. The 1-inch diameter tubing plugged off with what appeared to be cement, or gravel. Berry Well Service then ran a Jar and Stem tool down the well bore to try and determine where cement level was at. A good depth could not be determined with the Jar and Stem tool, Bill Mitten decided to shut down and will tag cement in the morning to determine cement height. |
