0221624788 2019-05-29 Plug / Plug Back No Violations On the day of my inspection I witnessed Berry Well Service had ran in the well bore with a Jar and Stem tool and tagged cement at 820 feet. Berry Well Service then ran 100 feet of 1.9-inch diameter tubing inside of the 8.625-inch diameter casing. Cemenco was on location and rigged up their cement lines and pumped 6.3 barrels, 30 sacks of 15.6 pound per gallon Class A cement with a yield of 1.18 cubic feet per sack with 5.2 gallons of mix water per sack and cemented to surface. After 25 minutes the plug was approximately 2 feet from surface. Bill Mitten with Berry Well Service informed me that they will let the top plug set for 25 hours and will return to perform the casing cut off.

0840245247 2019-05-28 Plug / Plug Back No Violations On the day of my inspection I witnessed Berry Well Service had ran 1945 feet of 1.9-inch diameter tubing inside of the 4.5-inch diameter casing. Cemenco was on location and pumped 6.3 barrels, 30 sacks of 15.6 pound per gallon Class A cement with a yield of 1.18 cubic feet per sack and displaced to 1,615 feet with 4 barrels of fresh water. Berry Well Service then tripped out of the hole and laid down the 1.9-inch diameter tubing Knox Energy then ran in the hole with a casing cutting tool to 1,320 feet and separated casing. Knox Energy then pulled 7 joints of 4.5-inch diameter casing to 1,090 feet and pumped 19 barrels, 90 sacks of 15.6 pound per gallon Class A cement with a yield of 1.18 cubic feet per sack and displaced to 769 feet with 12.5 barrels of fresh water. Berry Well Service then tripped out of the hole with the remaining 4.5-inch diameter casing and rigged down for the night. After speaking with Bill Mitten, he informed me that they would come back in the morning tag cement and determine where cement level was in the well bore.
0977750059 2019-05-22 Plug / Plug Back No Violations On the day of my inspection I witnessed Knox Energy had ran 2,015 feet of 1.9-inch diameter tubing inside of the 4.5-inch diameter casing. Cementco was on location and pumped 6.3 barrels, 30 sacks of 15.6 pound per gallon Class A cement with a yield of 1.18 cubic feet per sack and displaced to 1,615 feet with 4 barrels of fresh water. Knox Energy then tripped out of the hole and laid down the 1.9-inch diameter tubing Knox Energy then ran in the hole with a casing cutting tool to 1,320 feet and separated casing. Knox Energy then pulled 8 joints of 4.5-inch diameter casing to 1,069 feet and pumped 19 barrels, 90 sacks of 15.6 pound per gallon Class A cement with a yield of 1.18 cubic feet per sack and displaced to 769 feet with 12.5 barrels of fresh water. Knox Energy then tripped out of the hole with the remaining 4.5-inch diameter casing. Knox Energy then attempted to run 1 inch down the backside of the 8.625-inch diameter surface casing to 15 feet. The 1-inch diameter tubing plugged off with what appeared to be cement, or gravel. Knox Energy then ran a Jar and Stem tool down the well bore to try and determine where cement level was at. A good depth could not be determined with the Jar and Stem tool so Knox Energy shut down and will tag cement in the morning to determine cement height.

Knox Energy then ran 1.9-inch diameter tubing to 898 feet, Cementco was on location and pumped 6.3 barrels, 30 sacks of 15.6 pound per gallon Class A cement with a yield of 1.18 cubic feet per sack and displaced to 793 feet with 2 barrels of fresh water. Knox Energy then tripped out of the hole with the 1.9-inch diameter tubing to 100 feet and pumped 30 sacks of 15.6 pound per gallon Class A cement with a yield of 1.18 cubic feet per sack for the top plug. I spoke with Knox Energy representative Bill Mitten and he informed me that they will wait one day and make sure the cement for the top plug has not dropped and cut the casing 36 inches below ground level.

1515823331 2017-08-28 Production Wells No Violations This well is equipped with 4.5-inch diameter casing, a pumping unit, tubing, and rods. The pumping unit is connected to the rods. The pumping unit also has a motor and belts. The production area contains one 100-barrel production fluid storage tank, a 25 barrel production fluid storage tank, and a vertical mechanical separator. No production shown in RBDMS since 2014. The adjacent landowner claimed that the well shared a tank battery with permit number 34013205980000. He also stated that he did not believe anyone was receiving domestic gas from the well.
1541611829 2018-11-07 Production Wells No Violations Status check inspection found the well equipped with 4.5 diameter surface casing and 1.9 diameter production tubing with 2 diameter flow lines hooked to the casing and tubing well heads. Production valves are present and in the open position. Both flow lines join into a single 2 flow line near the well head after the production valves. The well is set up to be produced by the means of a pumping unit drive by a gas powered engine with a gas supply for the engine attached to the surface casing well head access port. The production facility is located along Patterson Hill Road on the opposite side of the road from the well and is shared with 3401320598-0000. The facility consists of a single 100 barrel steel production tank with 8 of standing fluid in it along with a 2,100 gallon plastic tank. Also present is a mechanical separator and gas sales meter with no current sales chart present. Dense vegetation is present at both the well head and production facility. Production records are current with the Division however no commercial production has been reported in paying quantities since 2014.
1558455218 2019-05-21 Plug / Plug Back No Violations A service rig owned by Knox Energy is rigged up on the well pulling the 1.900 production tubing and rods out of the well. A total of 59 joints of tubing and the mud anchor was pulled, the mud anchor was removed and the tubing ran back in the well open ended. The tubing was tallied. ODNR Inspector Ryan Armstrong was on site and will witness the plug job the following day. All calculations were reviewed.
1559049354 2019-05-28 Plug / Plug Back No Violations On site inspection to witness the beginning of the well plugging process accompanied by ODNR Inspector Ryan Armstrong. A service rig is on the hole owned by Knox Energy. The rig has pulled 1,884 of pumping rods out of the hole and has attempted to run additional 1.900 production tubing into the well. The 1.900 tubing was sitting down on bottom and the well was circulated by Cemenco Cement service before pumping a 30 sack bottom hole plug. The well did unload gas and oil when circulated with fresh water. Inspector Ryan Armstrongs will complete the plugging report and witness the entire plugging process.
