0824951625 2018-11-13 Production Wells No Violations On the day of my inspection I witnessed that production equipment included a 100-barrel production fluid tank, vertical separator and 2-inch meter run with drip all contained within an earthen dike. The well was equipped with 8.625-inch diameter surface casing, 4.5-inch diameter production casing and 1.5-inch diameter tubing and rods, with a General pumping unit and natural gas motor that was supplied through a drip on the production casing. At the time of my inspection all valves were closed, there were no identification tags present and production had been reported as zero since 2014. The Production area and well equipment were all surrounded by vegetation, and there was not a chart in the 2-inch meter run. Production was reported as 0 since 2014.

1318873049 2011-10-17 Preliminary Restoration No Violations A Preliminary Restoration check was completed at this site. This well is equipped with a pumping unit and the well is in production. The tank battery is equipped with one 110 barrel oil storage tank, one 50 barrel brine tank and one mechanical separator. This site meets requirements of the ORC and the Preliminary Restoration is now passed.
1559664152 2019-06-04 Plug / Plug Back No Violations On site inspection to witness the well plugging process with Knox Energys service rig on the hole and Cemenco Cement Services. The 1.900 tubing is set at 1,950 and Cemenco established circulation to surface before pumping a 6.3 barrel cement plug consisting of 30 sacks of class A cement weighing 15.6 pound per gallon and with a 1.18 yield. The cement was displaced to 1,550 with 3.6 barrels of fresh water. The 1.900 tubing was pulled out of the hole and the 4.5 diameter production casing will be ripped.
1559744411 2019-06-05 Plug / Plug Back No Violations On site inspection to witness the final stages of the well plugging process found a service rig on the hole owned by Knox Energy, the cement contractor is Cemenco Service. The 4.5 casing was ripped at 1,009 and one casing joint pulled up to 973. Cemenco pumped a 19 barrel plug to cover the rip and bottom of surface. The plug consisted of 90 sacks of class A cement with 3% calcium additives. The plug was displaced with 10 barrels of fresh water. The 4.5 casing was pulled out of the hole and the fluid level was confirmed at 100 before pumping a 6.3 barrel 30 sack surface plug. Cement stayed up to surface.
