At the production fluid storage area a single 100 barrel steel tank is present with fluid level of 7 observed, also present was a 1,200 gallon plastic tank found empty along with a mechanical separator and gas sales meter with no current sales chart present and no signs of recent activity around any of the well equipment and no well identification is posted. Instructions are as follows The well owner/operator shall submit a written plugging schedule to (Bill Stolarik) at (5880 Memory Road Zanesville ,Ohio 43701) within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. |

Attached to the casing and tubing well heads is 2 diameter flow lines with both check and shut off valves present on the commercial outlet well head ports. The casing valve is closed and the tubing valve is open. However on the casing head service port a 2 x 1 reducing fitting has been added along with a 1 diameter low pressure regulator that has a 1/4 brass line inserted into the tubing head, a 2valve is present and open. With the tubing head outlet line open gas is carrying over into the tubing head production flow line and into the mechanical separator at the production facility. All the separator valves are open along with the valves on both sides of the gas sales meter, the meter chart is outdated and appears to be from 2013. The well has gas pressure at surface and into the production facility however the main tie in point into the sales system near the public road has a valve present that is closed. No additional line was present to indicate domestic use has been added. At the production facility a 100 barrel steel tank is present with 76 fluid volume noted. There is also a plastic 2,100 gallon tank that appears to be empty. The well is set up to be produced by the means of a pumping unit. Dense vegetation is present at the well head and tank battery and no sign of recent human activity is present in the area. No identification is present. The last reported commercial production to the Division was in 2004. |
