There was a National pumping unit that was attached to the rods. There was a motor present on the pumping unit but it did not have any belt attached. The top valve on the production flow line was in the open position. The bottom valve on the production flow line was in the closed position. There was an odor of natural gas present around the well head. There was also an audible gas leak present. The production area contained one 100-barrel production fluid holding tank, one 50-barrel plastic production fluid holding tank, a vertical mechanical separator, and a charting box. The RBDMS system shows the last reported production on this well to be in 2004. There are no production records shown after that year. Photos were taken on the day of this inspection.

1458672333 2016-03-22 Production Wells Violations Noted Compliance notice follow up inspection found the following equipment present at the well head 4.5 production casing-1.5 production tubing and pumping rods with the well set up to be produced by the means of a pumping unit with no motive power present on the pumping unit, the 4.5 casing valve was closed on the commercial side. At the production fluid storage area a single 100 barrel steel tank is present with fluid level of 7 observed, also present was a 1,200 gallon plastic tank found empty along with a mechanical separator and gas sales meter with no current sales chart present and no signs of recent activity around any of the well equipment and no well identification is posted. Instructions are as follows The well owner/operator shall submit a written plugging schedule to (Bill Stolarik) at (5880 Memory Road Zanesville ,Ohio 43701) within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

1542816810 2018-11-21 Production Wells No Violations Follow up inspection to Chief Orders # 2017-356 & 2014-138 along with violation # 1458672333 fond the well equipped with 4.5 diameter production casing and 1.900 diameter production tubing with casing and tubing well head sections present. Attached to the casing and tubing well heads is 2 diameter flow lines with both check and shut off valves present on the commercial outlet well head ports. The casing valve is closed and the tubing valve is open. However on the casing head service port a 2 x 1 reducing fitting has been added along with a 1 diameter low pressure regulator that has a 1/4 brass line inserted into the tubing head, a 2valve is present and open. With the tubing head outlet line open gas is carrying over into the tubing head production flow line and into the mechanical separator at the production facility. All the separator valves are open along with the valves on both sides of the gas sales meter, the meter chart is outdated and appears to be from 2013. The well has gas pressure at surface and into the production facility however the main tie in point into the sales system near the public road has a valve present that is closed. No additional line was present to indicate domestic use has been added. At the production facility a 100 barrel steel tank is present with 76 fluid volume noted. There is also a plastic 2,100 gallon tank that appears to be empty. The well is set up to be produced by the means of a pumping unit. Dense vegetation is present at the well head and tank battery and no sign of recent human activity is present in the area. No identification is present. The last reported commercial production to the Division was in 2004.
