0218599033 2016-04-29 Production Wells Violations Noted On the day of my inspection I was following up on Chiefs Order 2014-138. The condition and status of the well appears to be the same. The well was equipped with a pumping unit, tubing, and rods. The tank battery consisted of a 100 barrel production fluid holding tank, a brine storage tank, and a mechanical separator. Production records have not been submitted since 2004. Photos were taken on the date of this inspection. The well owner/operator shall submit a written plugging schedule to Inspector Andrew Corder at 5880 Memory Road Zanesville, Ohio 43701 within 30 days, per OAC 1501:9-11-OS(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

1554127088 2018-10-17 Production Wells No Violations Status check inspection as a follow up to Orders number 2017-356 & 2014-138 and violation number 1699925709 issued on 05/19/2016. Upon inspection of the well head the well head and pumping unit was observed to be overgrown with vegetation, the well was equipped with 4.5 diameter production casing and 1.900 diameter production tubing with both tubing and casing well heads present. 2 diameter flow lines are attached to the casing and tubing well head with check valves and shut off valves present with the shut off valves in the open position. Both flow lines are tied into one steel 2 diameter flow line that leads to the tank battery separator. The well is set up to be produced by the means of a pumping unit with pumping rods inserted into the production tubing and a gasoline drive engine present on the unit. The production facility is in the area of the well head and consists of a 100 barrel steel construction storage tank that appears to be full of fluid. There is a 1,200 gallon plastic tank that has approximately 800 gallons fluid in it. There is a vertical separator present that has the flow line valve open feeding into it and pressure exiting the separator and into the gas sales meter. There is no current chart in the meter however all indications demonstrate the meter system has pressure present and the flow line is open. There is dense vegetation present around the production facility and the identification present represents Kerogen Resources. ODNR last reported commercial production was in 2004 and the Division filed for Bond forfeiture on 06/20/2005 and received the payment on 06/27/2005. No current Bond or Insurance is in place for the registered owner Kerogen Resources owner # 3054. The landowner indicated the well is leaking gas and I could smell gas around the well. The landowner also indicated the registered owner was paid to plug the well when they met in agreement regarding the Utica Shale rights.

1699925709 2012-01-23 Production Wells Violations Noted On the day of my inspection the well was equipped with a pumping unit. The tank battery contained one 110 barrel oil storage tank, a 50 barrel brine storage tank, and a mechanical separator. Well has the capability of being productive, however the valve is closed by the well head. This well has not had any production records submitted since 2004. This owner has forfeited his bond. Photos documenting my inspection were taken by me on the day of this inspection.
