1278171359 2018-08-01 Production Wells No Violations On the day of my inspection accompanied by ODNR inspector Bill Stolarik I witnessed that the well was equipped with 8.625-inch diameter surface casing and 2.375-inch diameter production casing. It did not appear to have any artificial lift technology. The production casing was no longer hooked to the flow line and both valves were closed at the time of my inspection. The Tubing had a 1-inch diameter rubber line tied to the flow line and the gate valve appeared to be open. Production equipment included a 100-barrel steel production fluid storage tank and horizontal separator. At the time of my inspection the 100-barrel tank appeared to have approximately 50 barrels of fluid contained within it. I informed Jim Corp when I arrived at the next location that a water truck was needed to pump the fluid off of the tank before it could be removed.
1536858846 2018-09-13 Production Wells No Violations Status check inspection found the well equipped with a 8.625 diameter surface casing string and a 4.5 diameter production casing string with a 2.375 upset tubing string present. There is a valve attached to the tubing string that is connected to a 2 diameter flow line. No mechanical or artificial lift technology is present. The production facility consists of a single 100 barrel capacity steel production tank along with a horizontal mechanical separator, a gas sales meter is present however the sales chart is not current. The well owner has applied for and received a plugging permit.
1539188420 2018-10-10 Plug / Plug Back No Violations On this day I arrived on location at 8:30 am and witnessed R&B Drillings rig on the hole and Formation Cementing on location with their pump truck attempting to was through an obstruction inside the 4.5 production casing at 2,650. The well was also circulating out the 8.625 surface casing while attempting to was the obstruction out. The company had 1.900 tubing in the hole and was unable to get past the obstruction at 2,650. Telegraph could be felt at surface when they sat down. I believe there is a solid bridge plug in the hole. Total depth is 3,920. I spoke with Cory McCrudden with Northwood who was the company representative on site and informed him they needed to make an attempt to remove the obstruction and get to bottom. I also followed up with a phone call to my direct Supervisor Brent Bear informing him of the directions given to the company.
1539610096 2018-10-15 Plug / Plug Back No Violations On this day I stopped by the location to check on the progress of the removal of a bridge plug set in the well bore at a measured depth of 2,650. A service rig owned by RD Drilling is rigged up on the well however the rig is not working today. It was reported that the rig worked Friday 10/12/2018 and attempted to remove the plug with a stem bit and jars. The company indicated they were unsuccessful.
1540317874 2018-10-23 Plug / Plug Back No Violations Status check inspection on the well plugging process found Northwood Energy has a service rig contractor identified as RB Drilling on location. The rig is attempting to drill out a bridge plug set in the well bore at 2,650. They have a mud pump and power swivel on location, both units are in dis repair and the service rig contractor is having trouble with the units. The service rig contractor has been notified to address the circulating and power unit leaks. There is also a hole in the 4.5 production casing causing the circulating fluid to discharge out the 8.625 surface casing at surface.
1540404583 2018-10-24 Plug / Plug Back No Violations On site inspection to witness RB Drilling attempting to remove an object out of the 4.5 production casing at a depth of 2,650 using a mud pump and power swivel with a 3.750 tooth bit. The rig has drilled for the 2nd day with no cuttings returned to surface and no hole being made deeper. I spoke with ODNR Supervisor Mike Moreland and Bill Arnholt with Northwood Energy. The rig is going to pull the drill string to inspect the bit and Northwood will submit a proposal to move foreword from this point. The State RBDMS records reflect the well was perforated between the depths of 2,278 and 2,538 with the total depth of 3,900.
1540562567 2018-10-26 Plug / Plug Back No Violations On this day I arrived on location at 8:00 am to witness the bond log and shooting off of the 4.5 production casing with a service rig on the hole owned by RB Drilling and Appalachian wireline service. A bond log was ran and the casing was attempted to be shot off at 1,016 unsuccessfully. A second attempt to shoot the casing off at 803 was successful with a top of bond showed at 836. The log also showed possible holes in the pipe below the bottom of the 8.625 surface casing at 870. The 4.5 casing was pulled out of the hole and 1.900 tubing was ran back in the well bore to the point it sat down at 896. The rig worked the tubing but it would not go past that point. Formation Cementing then pumped a 60 sack plug from 896 to 696. The tubing was then pulled up to 200 and a 60 sack surface plug set. The plug stayed up to surface.
1560258190 2019-06-11 Final Restoration No Violations Final restoration status check inspection found the lease road well site and production facility all need final grading and vegetation established. There is a large dirt pile near the entry point of the lease road that need addressed. The well was plugged on 10/26/2018. Instructions are as follows. Complete the restoration within 30 days, at the time of the restoration all final grading must be complete with an attempt to establish vegetation made. Erosion control measures must be made on the lease road area. An attempt to contact Bill Arnholt with Northwood Energy was made on 06/12/2019,, a voice mail was left regarding the completion of the restorations. Contact must be made with the ODNR County Inspector Bill Stolarik within 7 days of receiving this notice.

1575180181 2018-10-25 Plug / Plug Back No Violations On site inspection to witness the well plugging process with a service rig contractor on location identified as RD Drilling and Formation Cementing. Previous attempts to remove an obstruction inside the 4.5 diameter casing were unsuccessful and after 2 days drilling with a power swivel the Division granted the company to plug the well from the point they were at. The rig ran the 2,375 diameter 8 round thread upset tubing in the well to a depth of 2,595 and a 45 sack class A plug was set to cover the perforations between 2,538 and 2,278. The well circulated during the cementing process. There is a hole in the 4.5 causing the well to circulate up the backside of the 4.5 and out the surface casing well head. Upon setting the plug the rig pulled the tubing and the well will be logged the next day so the top of cement can be determined and the 4.5 shot off and recovered.
