0243827782 2013-04-23 Production Wells No Violations On the day of my inspection the well was equipped with a pumping unit and capable of production. The tank battery contents included one 100bbl tank, brine storage tank, and a mechanical separator. No violations noted.
0762686312 2018-11-07 Production Wells No Violations Status check inspection found the well equipped with 4.5 diameter production casing and 1.900 diameter production tubing with 2 diameter production flow lines present that are attached to the casing and tubing well heads. Production valves are present in the 2 diameter flow lines and in the open position at the well head. The well is set up to be produced by the means of a pumping unit driven by a gasoline engine with a natural gas supply to drive the engine attached to the 4.5 casing well head. At the production facility a single 100 barrel steel tank is present along with a 2,100 gallon plastic tank with a mechanical separator and gas sales meter also present in the dike area there is no current chart in the meter and the 2 sales line past the meter observed to be in the closed position. The tank battery is shared with 3411120339-000. Dense vegetation is present at the well and production facility. Production records submitted are current however no commercial production has been reported to the Division since 2014.

1426484930 2018-10-17 Production Wells No Violations On the day of my inspection I witnessed that production equipment included a 100 barrel production fluid tank, 37 barrel plastic brine tank, vertical separator and 2 inch meter run with drip all contained within an earthen dike. The well was equipped with 8.625 inch diameter surface casing, 4.5 inch diameter production casing and 1.5 inch diameter tubing and rods, with a Cabot pumping unit and natural gas motor that was supplied through a drip on the production casing. At the time of my inspection all valves were open, there were no identification tags and production had been reported as zero since 2014. The Production area and well equipment were all surrounded by vegetation, and there was not a chart in the 2 inch meter run.

1559570114 2019-06-03 Plug / Plug Back No Violations On site inspection to witness the final stage of the well plugging process with a service rig on the hole owned by Knox Energy and Cemenco Cement Service. Historically on 05/31/2019 a bottom hole and bottom of the surface pipe plugs were set. A 300 plug was set across the bottom of the surface casing through the 4.5 diameter production casing, the plug bottom was at 930 and the plug was tagged on this day at 632. Also a hole was discovered in the 8.625 surface casing on 05/31/19 while setting the bottom of the surface plug. Fluid level was tagged on this day at 305. The Pittsburgh # 8 coal seam was confirmed in the area of the hole. No cement appears to be on the back side of the 8.625 from the #8 Pittsburgh coal up to surface. Cemenco pumped hole volume and annular capacity 30.7 barrels of cement. Cement did circulate up the annular side but fell back down to 61 from ground level inside the 8.625. Cement volume pumped was 30.7 barrels consisting of 150 sacks of neat cement. The well will be allowed to sit and the cement cure before topping the annular and inside of the 8.625 off from surface.
1559606183 2019-06-04 Plug / Plug Back No Violations On this day a 2nd attempt was made successfully to set a surface plug. The previous days plug was tagged at 171 and a bridge was built in the well back up to 100. 40 sacks of class A cement was used. Cement stayed up to surface. The annular space was topped off also. Plugging was complete on this day.

1913469416 2017-08-28 Production Wells No Violations This well is equipped with 4.5-inch diameter casing, a pumping unit, tubing, and rods. The pumping unit is connected to the rods. The pumping unit also has a motor and belts. The production area contains one 100-barrel production fluid storage tank, a 25 barrel production fluid storage tank, and a vertical mechanical separator. No production shown in RBDMS since 2014. The adjacent landowner claimed that the well shared a tank battery with permit number 34013205980000. He also stated that he did not believe anyone was receiving domestic gas from the well.
