1541516752 2018-11-06 Production Wells Violations Noted Status check inspection accompanied by ODNR Inspector Ryan Armstrong. The well was observed to be equipped with a surface casing well head with 4.5 diameter production casing inserted into the surface casing. The top collar on the 4.5 diameter casing was equipped with a 4 x 2 reducing swage with a 2 ball valve on the swage in the closed position. The production tubing has been removed and the flow lines disconnected. On location near the well head is the production facility that consists of a single steel construction 100 barrel tank along with a mechanical separator. No gas sales meter or well identification is present. The 100 barrel tank has a large hole in the side that appears to be caused by construction equipment. Fluid can be seen present below the hole in the tank. The electrical service has been disconnected and no commercial production is on record as being reported to the Division. No domestic gas supply was observed as being present. Instructions are as follows. The well owner/operator shall submit a written plugging schedule to Inspector Bill Stolarik at 5880 Memory Road Zanesville, Ohio 43701 within 30 days, per ORC 1509:13(B)(7), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

The production tubing has been disconnected and the production flow line removed. On site near the well head is a 100 barrel production tank that has a hole in the side that appears to of been mad by a bull dozer blade. A plugging permit has been issued to the well owner by the Division and the well is slated to be plugged in the next 60 days.

1952735312 2018-04-17 Production Wells No Violations On the day of my inspection I witnessed the well was equipped with 8.625-inch diameter surface casing, 4.5 inch diameter production casing with a 4 inch by 2 inch reducing swedge, there was a 2 inch diameter valve that was closed and plugged. Production equipment included a 100 barrel tank, vertical separator, and drip enclosed in an earthen dike. Identification tags were not present, production had never been reported on this well. Well did not appear to be able to produce.
