0249106575 2019-04-16 Production Wells No Violations Compliance notice # 1541088618 follow up inspection accompanied by ODNR Inspector Rick Brown. The well is equipped with a surface casing well head installed on the 7 diameter surface casing with 4.5 diameter production casing installed into the well head. There is a 4.5 diameter frack valve on the well head in the open position. Installed onto the 4.5 diameter production casing is a 4 x 2 reducing swage which has a 2 diameter sales line attached. The sales line valve is open and is also tied into the surface casing well head before entering a sales line with (2) little joe pressure reducing regulators that are installed in the outlet sales line downstream of the well head assembly. No know gas sales meter or production equipment is present in the area, no house gas usage source was identified. The well did have a pressure valve on the 4.5 diameter casing indicating 80 pounds pressure present. No identification is posted. The company has not responded to the previous violation. Photographs were taken on this day.
1541088618 2018-11-01 Production Wells Violations Noted On the day of my inspection I found the well completely engulfed in vegetation with no well owner identification visible. The well was sitting inside a board fence enclosure with a tin horn cellar erected around the well and the surface casing well head below ground level elevation. The surface casing well head did have 4.5 diameter casing inserted into it. The 4.5 diameter casing was equipped with a 4.5 diameter 2000 psi Demco gate valve that is affixed inline to the top of the 4.5 diameter casing string. Atop the Demco valve is a 4 x 2 reducing swage that feeds into a 2 diameter sales line with a pressure gauge present reading 70 pounds pressure. The 2 diameter sales line is fed back into the surface casing well head with a 2 valve present in the open position. No check valve is present in line to isolate the surface casing or determine gas flow direction. The 2 sales line was reduced to (2) 1 diameter lines that run parallel to one another, the lines are equipped with model 041 Rockwell 3/16 orifice 10 to 25 pound working pressure Rockwell pressure regulators, downstream from the manifold both lines connect back into a single 2 diameter sales line that is buried under ground and not visible outside the well head area. No transportation line was found associated with the well. The landowner Robert Higgs indicated his Great Grandmother attempted to use the well for domestic purposes when the well was first drilled in 1978 and the well did not provide enough gas for the residence at that time and has not been used since. Photographs take on this day. Instructions are as follows. The well owner/operator shall submit a written plugging schedule to Inspector Bill Stolarik at 5880 Memory Road Zanesville, Ohio 43701 within 30 days, per ORC 1509:13(B)(7), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
