0340960322 2015-05-05 Production Wells No Violations Follow up inspection conducted in relation to inspection # 1429878982. The area of concern has been remediated and is now in satisfactory condition. Compliance Notice is now resolved.
0726796361 2018-10-01 Production Wells No Violations Status check inspection found the well equipped with 2 diameter production tubing and a tubing head only present above ground level. There is a riser pipe with a pressure relief device next the well head that appears to be attached to the casing well head that is not above ground level. The well is set up to be produced by the means of a pumping unit with a electrical drive motor. On location near the well head is the production facility equipped with (2) production tanks, one plastic capable of holding 80 barrels and one steel capable of holding 100 barrels. During previous inspections the producer has been cited for pollution and contamination due to production fluid being discharged and on this day a brine scald was observed - measured and photographed below a drain outlet associated with the production facility. The scald area measured 16 x 16 and was Quantab tested for chlorides with a high level Quantab strip test and showed 1500 parts per million chlorides. Photographs taken on this day and I spoke with my supervisor Brent Bear regarding the scald area below the production tanks.
1348489196 2012-09-24 Production Wells No Violations Inspection and identification per request of producer-Jim Yontz to identify permit number so He can add welll to His bond. Insprction found older producing well with no enviormental issues.Producers Identification # is 2356.
1427123541 2015-03-23 Production Wells No Violations Status check to assist inspector Andrew Corder in response to a complaint received by the landowner George Lafaber. Upon inspection on this day the following was found at the wellhead 6.625 casing 2 tubing and pumping rods with both the production and tubing valves open and production flowlines present. At the production fluid storage area a single 100 barrel tank was found present with a fluid separator located on top of the tank. In response to the complaint of pollution & contamination a discharge flowline was observed coming off the fluid separator and feeding into a 3 black in color corrigated line. At a measured point of 62 from the tank in a defined ditchline a 3 black in color corrigated line was found at a GPS point of 39.87166 x 81.08766 with stressed vegetation and visible scalding present to a measured point of 167 down the ditchline below the discharge point of the 3 line. Quantab Chloride low @ high level field strip tests were taken at the entry point and readings were found to exceed both Quantab strip tests, at the last measured point in the ditchline readings were also taken and found to exceeded the low level Quantab strip tests and read 5,737 parts per million on the high level Quantab strip tests. Photographs were taken on this day.
1428409161 2015-03-23 Production Wells No Violations On the day of my inspection I was accompanied by Inspector Bill Stolarik. The site had contamination that needs to be addressed. This well currently has a historical owner. Following registration the other issues will be addressed. Refer to Bills report for technical data concerning area of contamination.
1429878982 2015-04-24 Production Wells Violations Noted I spoke with Jim Yontz and expressed my concern with the affected areas. I asked him to visit the site and call me back with a proposal to remediate the affected areas. He said he would visit the site today. I also asked if he registered the well and he stated that Mr. Yoss, his attorney, sent all that information in. I let him know that dispersal was acceptable under Mississippian Exempt law however we had to be sure that this well meets all the criteria. Also, the operation can not cause pollution and contamination. Operator shall put a plan together and enact it to avoid further action.
1432297498 2015-05-20 Production Wells No Violations I left a voicemail with the owner to discuss the Mississippian Exempt Law. (08:00) Landowner claims this well does not meet one of the criteria. Landowner claims this well does not provide enough gas to run a pilot light. They have to use propane to run the appliances in the residence. The water is still being dispersed onto the ground at this time. I will try to contact Jim Yontz again to discuss this matter.

1539613516 2018-10-15 Production Wells No Violations Status check inspection as a follow up to my last inspection to confirm the full tank of brine has been removed off site. There was an oil haulers receipt dated 10/15/2018 indicating 81 barrels of brine has been picked up. The well owner has a service rig backed into the well but not rigged up.
1543863416 2018-12-03 Production Wells No Violations Status check inspection accompanied by ODNR supervisor Mike Moreland found the well equipped with only 2 diameter production visible above the ground level. There is a pressure relief device also present next to the well head. No domestic hook up is present. The well is set up to be produced by an electrical driven pumping unit. On location near the well is a production fluid storage facility consisting of (2) 100 barrel capacity production tanks one plastic and one steel. The tanks are situated inside a earthen dike area. Proper well identification s posted. There is an area of stressed vegetation below the production facility, the area was Quantab tested for chlorides with a low level Quantab strip with the strip test indicating 247 parts per million chlorides. The producer Jim Yontz was contacted and asked to establish proper vegetation in the area as weather permits.
1651866048 2015-06-08 Production Wells No Violations This complaint was referred to Jake Glascock to determine the extent, if any, of pollution. Updates to follow.
1670753042 2016-03-29 Production Wells No Violations On this day I received a call from Jake Glascock. The landowner had called him and was concerned with the operator and why he was installing a poll by the well. I visited the well site and spoke with the operator and during the wind storms his power poll became damaged. He was replacing it. I let the landowner, Virginia Lafaber, know what was going on.
1719364986 2015-06-18 Production Wells No Violations I contacted Jim Yontz again regarding the supply of domestic gas. Jim informed me that he himself laid the line to the house which included a drip tank. He said that the landowner used it for a while and then stopped because they did not want to deal with the issue of having to thaw out the line during the winter months. The landowner has propane on the property now. He said that the gas is still there and available for domestic consumption.
