0129076937 2015-02-04 Plug / Plug Back No Violations Follow up to 10948305. Restoration requirements met. I spoke with the pumper from petrox and he informed me the line running past this well is active.
0175946197 2011-08-05 Administrative Inspection No Violations Clay allowed to settle while crew rigged down. Clay level dropped 30 feet, so it was topped off before they left.
0215873422 2011-08-08 Plug / Plug Back No Violations Stopped by site to see of clay level had dropped over the weekend. It was still at the surface. Chipco will be capping off well today.
0296802281 2011-08-02 Administrative Inspection No Violations Received call from Justin Smith of Chipco letting me know they pulled 19 joints of 8 5/8 inch surface casing then shoveled 6 tons of clay into surface casing borehole. They then tagged the top of the clay plug at 673 feet.
0395402179 2007-07-25 Production Wells No Violations No violations were found at the time of inspection.
0467356301 2011-07-19 Plug / Plug Back No Violations Steaming out inside of tubing in order to shoot it off. Tools only go down to appx 2700, because of paraffin.
0472775441 2011-08-01 Plug / Plug Back No Violations Drove to site to witness pulling of 8 5/8 inch casing. No trailer was on site for rig crew to load with casing. Waited for roughly an hour to hear from Chipco about whether or not a trailer would be coming today. No trailer coming.
0803227774 2011-07-27 Plug / Plug Back No Violations Witnessed clay plug being shoveled from 7 seat up past Berea, roughly 750 shovels. Since cement bottom plug came 600 feet into Big Lime it was allowed to act as Lime Plug as well. Will begin pulling 8.625 tomorrow.

1222207948 2011-07-26 Plug / Plug Back No Violations Cement top tagged at 2876. 4.5 casing ripped at 2782.
1311803871 2011-07-22 Plug / Plug Back No Violations Casing cutter used to cut tubing. Steam used to allow cutter down as far as possible. Shot off at 3167 feet. Asked Chipco to place cement from TD to 500 above rip.
1312218549 2011-07-29 Plug / Plug Back No Violations Ripping 8 5/8 inch casing at 521 feet.
1312477320 2011-08-04 Plug / Plug Back No Violations Witnessed clay being washed down surface casing borehole. About 8 tons in hole today, water level rose to top. Allowed to settle and only dropped 5 feet. Tagged top of clay plug at 150 feet. Will allow to settle overnight and top off tomorrow.
1312554165 2011-08-05 Plug / Plug Back No Violations Water level was 100 feet and clay plug was tagged at 145. Shoveled remaining 145 feet of clay plug into hole. Will cap off Monday 8/8/11.
1312911607 2011-07-22 Plug / Plug Back No Violations Witnessed plugging operations with Josh Garland. DMPW Oil Field Steam was steaming the tubing to get through the paraffin in the tubing. Was unable to get wire line below 3180 feet. Appalachian Well Surveys Used casing cutter to cut tubing at 3167.5 feet.

1940457129 2013-05-02 Plug / Plug Back No Violations Follow up to 10948305. A large scalded area roughly 100 x 20 feet still remains near the well. There is significant erosion along the lease road. There is still production equipment at the site.
1941658198 2014-05-05 Plug / Plug Back No Violations Upon inspection, it was observed that the former well site and access road are still mostly barren and are causing significant erosion. There are still flow lines in place that run from the former well site approximately 1/2 mile back to the tank battery. These flow lines need to be removed and the entire site needs to be stabilized with a vegetative cover before this restoration can be passed. I will contact Chipco and inquire about a prospective timeline to get this final restoration completed.
