Inspection Data

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
1469545376 2016-07-26 Exempt Well Pre Transfer No Violations I inspected this well location pursuant to an intended transfer for exempt domestic use. At the time of this inspection, I observed the following: a wellhead and associated tank battery located on a rural property. The well is comprised of a mechanical plunger lift system with a 2-inch flow-line and a 1.375-inch domestic supply connection. The battery is comprsied of one 100-barrel steel production tank, one 50-barrel steel brine tank, and horizontal separator. Connections at the well and bettery were secure and free from signs of leakage or problems. The tank load-line connections were plugged and secure. There was estimated to be less than 10 barrels of residual fluid in the production tank, and less than 5 barrels of residual fluid in the brine tank. Secondary containament was present at the battery, but most of the interior portion was overgrown significantly with vegetation. The unpaved access road to the battery was passable. Beyond the battery the access road was overgrown, and generally passable on foot or all-terrain vehicle only. I spoke with Mr. Glen Bojanowski, intended exempted domestic well transferee. He indicated that he owns the parcel on which both the well and tank battery were located , and was currently receiving domestic gas from the well. I confirmed that Mr. Bojanowski was listed as the owner of Ashtabula parcel # 540200000200 as recorded by the Ashtabula County Auditor. Mr. Bojanowski indicated that all of the existing surface production elements inlcuding the tank battery system and wellhead plunger lift mechanicals were included in the ownership transfer and would remain. I did not identify any outstanding regualtory compliance issues.
