1237052015 2018-04-02 Plug / Plug Back No Violations I conducted a compliance notice follow-up inspection for compliance notice 1428333039. At the time of this inspection, the well consisted of the 8 5/8 inch surface casing riser, open at the ground surface. No gas was observed to be present on the open well annulus. The former tank battery location still remains unrestored. There had been no apparent attempt to perform any of the final restoration work as required in the previous inspection reports.

1428333039 2015-03-30 Final Restoration Violations Noted I inspected this well to determine the status of the final restoration. My inspection found a pile of stone in the old tank battery area. There is a two inch steel pipe above ground level located on north side of old tank battery area. I walked to the well location which is located in a crop field. I found the surface casing head above ground level with boards sticking in the well bore. The boards were five foot long. There was a solid structure three feet down the well bore. The well bore was full of fluid. There is small gas bubbles venting through the fluid. This well is not in compliance with 1509 of the Ohio revised code. Remedial work required to get well into compliance: REMOVE STONE FROM TANK BATTERY AREA, REMOVE FLOWLINE OR SUBMIT A LANDOWNER WAIVER FOR THESE ITEMS, DRILL OUT THE WELL BORE AND RE-PLUG WELL.
1436984234 2015-07-15 Plug / Plug Back No Violations I was on site to take photos of the violations [ leaking well, gravel at tank pad, flow line at tank pad ]. I took photos of the pile of gravel at the tank pad area, flow line at the tank pad area and the well head and down the well bore. I spoke to the landowner [Dushan Kalvznik ]. I ask him about waiving the flow line and the pile of gravel at the tank pad area. He told me he will not waive anything relevant to the well.

As such, the following deficiencies had been noted previously: leakage of gas had been noted from the previously-plugged well, at the surface casing, which had been left open at the ground surface. In addition, final restoration work at the wellhead and former battery location had not been completed. At the time of this inspection, I noted the following: a wellhead consisting of a 8.625-inch surface casing riser, open at the ground surface. Upon inspection, I did not note any gas in the well annulus which was full of water. The former tank battery location remained unrestored. There had been no apparent attempt to perform any of the final restoration work as required.

Inspection of the well found there was fluid in the surface casing at ground level. There was gas venting through the fluid at small but steady amount. There has been no attempt from the company to reclaim or re-plug this well.
