0519952046 2014-09-05 Production Wells No Violations Complaint # 4755, David Catanese. My inspection found the oil saturated soil around the well head has been removed and soil has been replaced. The area has been seeded and mulched. A working emergency phone number has been posted on identification sign at the tank battery area. I spoke with Ed Griffin, he told me they had to order the charcoal filter for the production tank and when it arrives they will install it.

The tank battery consisted of one 210 BBL tank and a mechanical separator. The diked tank battery had legible identification. The lease access was stable.

1407239075 2014-07-25 Production Wells Violations Noted Complaint# 4755, David Catanese. I met with David Catanese and his wife at the tank battery area. I spoke with them to find they are smelling a strong odor of gas eminating from the tank battery area. While we were talking I would get a whiff of gas coming from the 210 barrel steel production tank every few minutes. They were also not saisfied with the maintenance of the tank battery area, [ cutting the tall weeds around the tank battery ]. I told them there was no action I could inforce for the company to cut the weeds down. There was a area on the 210 barrel steel tank that showed signs of a small leak located 7 from ground level on the west side. Further inspection of the well location found a pumping unit with fresh oil on the ground around the well casing in an area 4 x 8. I contacted the emergency phone number posted on the identification sign to get a recording saying the mail box was full and try again later. REMEDIAL ACTION REQUIRED TO CORRECT THE VIOLATIONS: Repair leak in 210 barrel production tank, install a charcoal filter on production tank, remove all oil saturated soil at well location and replace and establish vegetation growth, also post a working emergency phone number on the identification sign.
