0097751885 2017-09-21 Production Wells No Violations I inspected this producing well as a follow-up to a compliance notice that had been issued directing the owner to complete the following: repair defective 4.5-inch well casing that was actively leaking gas, remove stock tank leaking from heater tube, and remove impacted soils. At the time of this inspection I observed the following: there was gas actively leaking from the 4.5-inch well casing as had been previously observed with no evidence of attempts to repair. In addition, the leakage that had been observed from the 210-barrel stock tank remained with no signs or evidence that attempts to mitigate the impacted area had been implemented. 0150736574 2015-06-30 Production Wells No Violations My inspection found there has been a attempt to fix the leak from the heater tubes at the base of the production tank. There is still brine dripping from the heater tube at a much slower rate. No other action has been taken to correct the violations.
0246553929 2015-12-02 Production Wells No Violations I conducted an inspection of this well and tank battery and found there was still fluid leaking from the heater tube on the production tank. Pooled fluid had accumulated beneath the heater tube leak. The dike needs rebuilt to prevent escape of any waste or liquids. The signage has been added to the production tank.

There is still some oil saturated peat sorb remaining. The heater tube was still dripping fluid onto the ground. The fluid was not pooling which indicated the fluid is going below the surface. A earthen dike has not been established around the storage tank. A identification sign has not been posted at the well location.
1087600200 2017-09-12 Production Wells No Violations At the time of my inspection the heating tubes on the 210 barrel production tank were no longer leaking. Fluid level in the tank is now below the heater tubes. The 210 barrel production tank has not been removed and replaced or repaired as required in inspection report dated 04/01/2015. Brine scald is evident under the tank. Soil pollution and contamination is still present under the tank, no action has been taken by the owner to remove the soil, bio-remediate the location if possible or make any effort to correct the issues at the tank battery. The 4.5 inch casing is leaking and has not been repaired as required in the inspection dated 08/31/2016. No action has been taken by the owner to correct the Compliance Notices issued on 04/01/2015 nor the Compliance Notice issued on 08/31/2016.

1390214060 2017-11-01 Production Wells No Violations I inspected this well location as part of on-going enforcement proceedings. As such, the following deficiencies had been noted previously: leakage of fluids (oil and brine) from the heater tube located at the bottom of the 210-barrel steel production tank. Leakage of fluid had been observed to impact areas beyond the secondary continament on the north side of the battery. In addition, it had been noted that gas was leaking from the 4.5-inch production casing. At the time of this inspection, I noted the following: a wellhead with plunger lift system and associated battery on a larger, rural parcel. The well is located 200 feet west of the battery. The battery was comprised of one 210-barrel, steel production tank, and horizontal separator. Gas was freely flowing from a failure at the 4.5-inch production casing. It is evident that a previous attempt to repair the leak using a clamp device had failed. There was rust staining at the base of the production tank, near the heater tube location. I did not observe any active fluid leakage from the tank. Subsequent inspection of the tank indicated that there was no visible fluid remaining. The areas of brine scald that had been observed previously on the north side of the tank battery had become overgrown with vegetation, and were no longer visible. The chart recorder was not functioning. There was no visible evidence that any actions had been taken to remediate and/or repair the previously-noted problems and deficiencies.
1428340098 2015-04-01 Production Wells Violations Noted My inspection found fresh oil and brine on the ground at base of steel production tank three feet wide on north, south and west sides. There is fluid leaking from heater tube on west side of the production tank. There is fresh oil on the outside of the production tank on the north side from the top to the base of the tank. The earthen dike around storage tank is not of sufficient size to contain oil and brine stored in the production tank. The wellhead has a active domestic hook-up from the production casing. The identification sign has permit number 1257 posted on it. Permit number 1257 shares tank battery with this well which is permit number 1222. There is no identification posted at the well location. This well is in production at this time. ODNR Division of Oil & Gas records show the last reported production was in 2012 producing 3468 mcf of gas. This well is in non-compliance. Remedial work required to get this well into compliance: EMPTY PRODUCTION TANK IMMEDIATELY, REMOVE CONTAMINATED SOIL AROUND AND UNDER PRODUCTION TANK, REPAIR OR REPLACE LEAKING PRODUCTION TANK, ESTABLISH A EARTHEN DIKE OF SUFFICIENT SIZE TO CONTAIN THE CAPACITY OF THE PRODUCTION TANK, POST PERMIT NUMBER FOR THIS WELL ON IDENTIFICATION SIGN AND POST A IDENTIFICATION SIGN AT WELL LOCATION, SUBMIT PRODUCTION REPORTS FOR THIS WELL FROM 2012 TO PRESENT. I contacted Don Goosman and told him of the violations, he told me as soon as he has equipment he will perform the remedial work.
1436541969 2015-06-09 Production Wells No Violations Northeast Regional Supervisor Tom Hill and I were on location in reference to compliance notice #1428340098. Upon arrival, we found the well tender for Big Sky was on location. During our inspection, we found the contaminated soil on the north side of the 210 barrel tank has not been remediated. The contaminated soil stretches over the north dike wall. An earthen dike of sufficient size has not been built. I observed fresh oil still on the outside of the 210 barrel production tank on the north side from the top to the base of the tank. Rudder caps with hose clamps have been installed on the heating tubes. While on site, I told the well tender the highly faded identification sign needed updated with the correct permit number. The well tender obtained a black marker from his vehicle and corrected the entire identification sign. Next, we told the well tender that:



The well tender stated Big Sky was waiting on a part to finish repairs on the backhoe. Follow-up inspection will continue until all violations are corrected.

1441794244 2015-08-19 Production Wells No Violations I was on location for a follow-up inspection in reference to compliance notice no. 1428340098. During my inspection, I found the impacted soil around the 210 barrel tank scrapped up and removed. There are two rubber caps on the heating tubes. An identification sign is posted.
1446489887 2015-11-02 Production Wells No Violations My inspection found there was still fluid leaking from the heater tube for the storage tank. There was salt crystals on the ground underneath the leaking heater tube. There has not been a earthen dike of sufficient size established around the storage tank. There was no identification posted at the well location.
1472658445 2016-08-26 Production Wells Violations Noted On site with DOGRM Inspector Mike Dinsmore at the time of my inspection the 210 barrel production tank was continuing to leak at the heater tubes. The rate of leak has slowed since last site inspection, the tank is nearly empty. There was iron staining 3 feet in diameter around the heater tubes that was leaking. Rubber caps the operator has placed on the heater tubes were still in place. The well was shut in. The 4.5 inch casing has a metal clamp on it above the Braden head. The clamp has failed; the well was releasing gas from the 4.5 inch casing at a vigorous rate from behind the clamp. The following remedial action needs to take place at the location: 210 barrel production tank shall be removed and replaced. All contaminated soil from under the tank shall be removed and properly disposed of. Division shall receive disposal tickets from where the material is disposed of. The 4.5 inch production case shall be repaired in order to stop the uncontrolled release of gas into the environment.

The gas leak at well that was noted on a report by Tom Hill on 8-26-2016 has been corrected. I took one photo of the scalded area at the base of the production tank on the west side.

2105055350 2018-03-06 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 14283440098. At the time of this inspection, the well was equipped with a plunger lift assembly and the annulus and flow line valves were open. The 4.5 inch casing leak was repaired with a clamp. The tank battery consisted of one 210 BBL production tank and a mechanical separator. The dike around the tank battery still needs rebuilt and the impacted soil within the battery still needs to be cleaned up and remedied. Signage has not been updated at the well or tank battery location.
