0538026613 2015-08-19 Production Wells No Violations I was on location for a compliance notice follow-up inspection. During my inspection, I found the stockpiled soil from the oil spill not removed. There is a backhoe parked on top of the stockpile. The west end of the stockpile has small pools of oil. Photos were taken and available upon request.
0582837911 2017-10-30 Production Wells No Violations I inspected this well location as part of legal proceedings. As such, the following deficiencies had been noted previously: leakage of oil from the wellhead that had impacted the areas surrounding the wellhead and migrated to an adjacent tributary stream as well as a small, farm pond, as well as improper storage/disposal of oil-impacted soils, and clean-up materials, which had been stockpiled or otherwise left on site following mitigation and clean-up activity.. At the time of this inspection, I noted the following: a wellhead with closure swage and 2-inch hard-line flow connection located in a relatively level, open farm field. The flow-line valve was closed. There was a small pile of gravel and gravelly soils adjacent to the wellhead. There was no visible or discernible signs of oil leakage or impact in the areas surrounding the wellhead. The battery was comprised of one 100-barrel, steel production tank and horizontal separator, which was surrounding by mature grasses which obscured much of the battery. I did not observe any obvious signs of oil or impacted areas in or around the area of the tank battery, or along the course of the flow-line. I walked the areas surrounding the pond, and walked the upstream course of the small tributary stream, between the pond and the wellhead and found no discernible evidence of impacts. There was some portions of left-over storm culvert. There were several areas of disturbed soils from remedial excavation that require re-grading. I observed no other signs of discarded soil or remnant debris from clean-up activity. ID signage was present but not complete (lacking complete API permit #).

I did not observe any obvious signs of oil or impacted areas in or around the area of the tank battery, or along the course of the flow-line. There was a chart recorder that was not active. There were no signs the well was being produced. Areas previously disturbed by remediation activity had not been re-graded or restored completely. There was a discarded half-round steel tank that needed removed.

I had the company dig four test holes in the remaining stockpiled soil from the clean-up operations and could find no more oil saturated soil at this time. The remaining remedial work of reclaiming the disturbed areas from the oil spill clean-up should be done when the weather conditions permit the remedial work. NOTE: A total of 56 yards of material was removed from the oil spill site.

1171589355 2015-10-23 Production Wells No Violations I was on site to find the company had moved a steel half tank on site. Upon inspection of the tank I found several holes in the bottom of the tank. The company had planned to place the oil saturated soil in the tank to move it to the road to be removed from the site. I denied them from using the tank due to the integrity of the tank. I then left the site.
1394312027 2014-12-11 Production Wells No Violations I arrived on site to find the spill origin had a earthen dike around it. There was a brine truck by pond and workers were vacuuming the oil from the surface of the pond. There was also workers working down stream removing the oil.
1440689181 2015-08-26 Production Wells No Violations On site with Reggie Brown (OEPA) and Steve Tompkins (DOGRM Inspector), inspection of the spill area confirmed that the operator had started redistributing the soil stockpile from the oil spill clean-up operations, against direction of DOGRM, OEPA and Federal EPA. Reggie Brown (OEPA) indicated that additional soil redistribution had taken place after the operator was order to stop redistribution by OEPA. Operator has yet to submit a restoration plan to the DOGRM nor the OEPA, as agreed that Big Sky Energy Inc. would do on the inspection conducted 07/09/2015 (Big Sky Inc. agreed to submit a restoration plan by O9/15/15 to the Division for review.) During that inspection the operator was informed by DOGRM and OEPA that soil testing could be completed by the operator and if sections of the stockpile tested clean, the clean soil could be redistributed. No test results for soil sampling have been received by the DOGRM or the OEPA. Review of the affected pond during the inspection revealed that an oil sheen is still evident when the ponds edges are disturbed. Debris from the siphon dams constructed on the north edge of the pond remains on location. Debris from the first siphon dam in the stream remains on location, there is one culvert pipe still in the stream under the remaining portion of the dam. One culvert pipe on the bank of the stream and one further down stream. A sheet of plywood and t-posts remain from the second siphon dam in the stream. Brush, small trees and stumps that were removed and stockpiled during the initial clean-up effort are still on location and need to be addressed.
1458743221 2015-10-08 Production Wells No Violations I was on site to witness the pressure test on the flow line. The company pressured the flow line with the pressure from the well to 56 PSI. They held the pressure for 15 minutes with no loss of pressure. The pressure test passed.

The well and associated battery are located on a rural parcel. There was no visible or discernible signs of oil leakage or impact in the areas surrounding the wellhead, which were covered in several inches of snow. The battery was comprised of one 100-barrel, steel production tank and horizontal separator. The tank was empty. I did not observe any obvious signs of oil or impacted areas in or around the area of the tank battery, or along the course of the flow-line. There was a chart recorder that was not active. There were no signs the well was being produced.
1705775389 2016-03-18 Production Wells No Violations I was accompanied by Jen Barret, Dan Martin, Mike Williams [ Attorney Generals Office ], Rick Anderson and Tom Hill [ ODNR Division of Oil & Gas ]. Upon arrival we found there was a new chart on the gas meter with the date of 3-16-2016 wrote on it. The chart indicated the well was in production at the time of the inspection. We then walked to the oil spill location and discussed the reclamation that was needed. I took a photo of the chart on the gas meter.
1861835780 2016-09-12 Production Wells No Violations My inspection of the area that the soil from the oil spill clean-up had been stockpiled had been roughly graded, also the hole that the company created from building the syphon dam had not been filled in. The work that had been done was not sufficient enough to resolve the remedial work needed.
1871976658 2015-10-14 Production Wells No Violations I arrived on site to find there has been reclamation work done at the syphon dam location north-east of overflow of the pond and at the end of the stream where the oil had flowed. More reclamation is needed in both of these areas.
1875528880 2015-11-20 Production Wells No Violations I was on site to witness the removal of the oil saturated soil from the oil spill in December of 2014. The company was using a backhoe to move the soil from the oil spill location to a dump truck at Sheffield road. The dump truck then transported the soil to Big Skys yard located north of route 307 and east of route 11. The soil was placed on a plastic liner with a dike constructed from 8.625 steel casing surrounding the area. The dump truck was able to move four yards of material per load. I witnessed the removal of 24 yards of soil. I then left the site.
1890863746 2015-09-18 Production Wells No Violations I was on site to witness the excavation of the flow at the area the line had leaked. Upon the excavation of the gas line we found there was a 5 section removed from the gas line. The company workers then replaced the missing section of the gas line. We then closed the gas line in at the tank battery and the well and attempted to pressure test the line with the well pressure. They were unable to pressure up on the gas line to properly test the line. The company will test the gas line when enough pressure builds up in the well.
2068928948 2014-12-09 Production Wells Violations Noted I received a phone call from Darren Sable on 12-8-2014 in the evening. He told me the oil company that owns the well on his property had a oil spill which had entered a small pond and then traveled down stream for some distance. I met Darren at the well the next morning when he showed me the location of the origin of the oil spill. I witnessed oil on the ground in the lease road about 1000 south of the tank battery location. The oil covered an area 70X 20 and had traveled about 50 in a ditch where the oil had entered a small pond, the oil had then flowed to the overflow of the pond and entered a small stream. I walked down stream 800- 1000 to find small puddles of oil had been caught in debris in the stream in about 10 different spots. The amount of oil down stream was a very small amount. The amount of oil spilt from the flow line leak was about 2-3 barrels. Reggie Brown with the EPA then arrived and I walked with him to the oil spill origin. We discussed the clean-up procedures the company needed to perform to remediate the oil spill. The stream and pond clean-up was under EPAs authority. Bob Barr Jr. arrived and myself and Reggie Brown discussed with Bob the clean-up needed . The end result was to remove all oil from the spill origin, the pond and the stream and dispose of in an approved manner. Note: The coordinates of the spill origin are 41.82597, -80.63139 and the coordinates of the end of oil found in the stream are 41.82885, -80.63056..
