0112672850 2015-10-29 Production Wells No Violations My inspection found there was a sump hole dug underneath the unloading valve on the storage tank. There was absorbent pads floating on the fluid that has filled the sump hole. The oil saturated material inside the earthen dike walls has not been removed. The flow line has not been buried from the tank battery area to the well location. There was no identification posted at the tank battery location.
0209422615 2016-03-18 Production Wells No Violations I was accompanied by Jen Barret, Dan Martin, Mike Williams [ Attorney Generals Office ], Rick Anderson and Tom Hill [ ODNR Division of Oil & Gas ]. Upon arrival we found there was a new chart on the gas meter with the date of 3-17-2016 wrote on it. The chart indicated the well was in production at the time of the inspection. I took photos of the ground around the production tank and the chart on the gas meter.

0328071160 2017-10-30 Production Wells No Violations I inspected this well location as part of legal proceedings. As such, the following deficiencies had been noted: leakage of gas and oil at the tank battery and wellhead, and lack of complete identifying signage. At the time of this inspection, I noted the following: a wellhead with pump jack system and associated battery on a larger, rural parcel. The wellhead was surrounded and overgrown with tall, mature phragmites. I noted gas leaking from a 2-inch Balon valve at the production casing flow-line connection. I also noted oil and paraffin staining around the wellhead base. There was a black, plastic, 2-inch flow-line from the wellhead that was not properly buried. The battery was comprised of one 100-barrel, steel production tank and horizontal separator. I noted the interior areas of the secondary containment to be heavy impacted by oil. ID signage was present but not complete (lacking complete API permit #).

The well and associated battery are located on a rural parcel. At the time of this inspection, I noted the following: a wellhead with pump jack system in a clearing surrounded by tall grasses. The pump was idle. I noted gas leaking from a 2-inch Balon valve at the production casing flow-line connection. I also noted oil and paraffin staining around the wellhead base. The associated tank battery was comprised of one 100-barrel production tank, and horizontal separator. The tank was empty. Oil and oil-stained surface materials around the base of the tank from a previous release remained with no signs that attempts to remediate the spill had occurred. There were no signs that the well was being produced. There was a chart record box that was not active.

My inspection found the gas meter had a chart on it which had the date of 4-23-2014 posted . The chart also showed the well is in production . There is fresh oil around the 100 barrel production tank with salt crystals around a small puddle of water underneath the 3 valve . This tank appears to be leaking . Also there is not a legible identification sign posted for this well . This well is not in compliance with the Chiefs order # 2014-43 .

The flow line from the tank battery to the well has not been buried. The valve from the well to the flow line was closed. There was an active domestic hook-up piped from the tubing head. There was no identification posted for this well at the tank battery location.

1890669422 2015-08-19 Production Wells No Violations I was on location for a follow-up inspection in reference to compliance notice no. 1909460169. During my inspection, I found oil impacted soil around the 100 barrel production tank. There is no identification posted. I observed several trees on the north side of the tank battery are dead. The plastic flow line has not been buried. Photos have been taken and are available upon request.
1909460169 2015-07-10 Production Wells Violations Noted My inspection of this well found fresh oil on the ground around the 100 barrel steel production tank extending three to five feet from the base of the tank. There was no identification posted for the well. There was ten trees on the north side of the tank battery extending ten feet into a wooded area that are dead or dying. There was vegetation growing underneath the dead and dying trees. There was a two inch plastic flow line lying on the surface from the separator to the well through a wooded area. There was a active domestic hook-up from the well. REMEDIAL WORK REQUIRED TO CORRECT THE VIOLATIONS: REMOVE ALL OIL SATURATED MATERIAL INSIDE THE EARTHEN DIKE AREA, PROVE THE INTEGRITY OF THE ONE HUNDRED BARREL STEEL PRODUCTION TANK IS IN TACT TO PREVENT LEAKING OF THE FLUID STORED IN THE TANK, BURY THE FLOW LINE FROM THE TANK BATTERY TO THE WELL, ESTABLISH IDENTIFICATION FOR THE WELL.

At the time of this inspection, I noted the following: a wellhead with pump jack system in a clearing surrounded by tall grasses. The pump was idle. I noted a continued gas leak from a 2-inch Balon valve at the production casing flow-line connection. I also noted oil and paraffin staining around the wellhead base. Thee was an open and active domestic tie-in. The associated tank battery was comprised of one 100-barrel production tank, and horizontal separator. The tank remained empty. There were several inches of standing water in the secondary containment with visible floating sheen with free oil easily liberated from the underlying gravel ballast upon disturbance, and oil staining to surround soils. The flow-line connection was open at the wellhead but the chart recorder was not active. There were no signs that any attempts to mitigate or clean-up the oil impact or leaking gas at the wellhead had occurred.
