0127735196 2016-09-20 Production Wells Violations Noted My inspection of this well found there was a gas leaking from the base of the plunger catcher. The well casing above the ground was in a very deteriorated condition. The valve on the tubing and to the flow line was closed. There was an active domestic hook-up piped from the tubing head. REQUIRED REMEDIAL WORK TO RESOLVE THE COMPLIANCE ISSUES: Repair the gas leak at the well and address the deteriorated casings from the well at the surface.
0422864639 2018-03-06 Production Wells No Violations I inspected this well pursuant to Chiefs Order #2016-300 issued for lack of insurance. The well and associated battery are located on a rural farm parcel. At the time of this inspection, I noted the following: a wellhead with plunger-lift system in a wooded clearing. The well was idle with the exception of active domestic tie-in. There was some gas leakage from the production tubing which was severely corroded at the wellhead. The associated tank battery was comprised of one 210-barrel production tank, and horizontal separator. The tank was empty. The secondary containment was mostly overgrown with vegetation, but was free from signs of spills, or leaks. There were no signs that the well was being produced.
0780442010 2013-05-01 Production Wells No Violations My inspection of the flowline route from the tank battery to the well showed the line has been re-buried . Myself and supervisor Hill used a metal probe to check the depth of the flowline was the sufficient depth . We found one spot that the line was less than 1 deep , also the flowline route from the tank battery to the well needs final grading . I contacted Jamie per request from Bob Barr Jr. and told him of remedial work still needed to resolve violations [ bury flowline where I marked it with a orange flag to the sufficient depth and final grade flowline route ] .

0853089891 2015-10-29 Production Wells No Violations My inspection found that a logging company has logged the property resulting in the flow line route being sufficiently graded. The area at the beginning of the flow line route by the tank battery had some ruts for 40 plus feet due to heavy equipment from the logging company going through a wet area. This is not a compliance issue. Inspection of the well found the rabbit catcher was deteriorated to the point that there was gas leaking from the well. The identification sign at the tank battery location has faded to the point it was not legible. I contacted Charles Moisio [ well tender ], he told me he would repair the gas leak immediately and post the well information on the identification sign. The grading of the flow line route resolves the remedial work required by the original Compliance Notice issued on 8-21-2012. The new violations will be addressed upon a follow-up inspection for the gas leak.
0943108746 2017-10-30 Production Wells No Violations I inspected this well location as part of legal enforcement proceedings. As such, the following deficiencies had been noted: failure to properly bury the associated flowline. At the time of this inspection, I noted the following: a wellhead with pump jack system and associated battery on a larger, rural parcel. The wellhead was surrounded and overgrown with tall, mature phragmites. I noted gas leaking from a 2-inch Balon valve at the production casing flow-line connection. I also noted oil and paraffin staining around the wellhead base. There was a black, plastic, 2-inch flow-line from the wellhead that was not properly buried. The battery was comprised of one 100-barrel, steel production tank and horizontal separator. I noted the interior areas of the secondary containment to be heavy impacted by oil. ID signage was present but not complete (lacking complete API permit #).

1178904025 2017-09-12 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 127735196. The well was observed to still be leaking from the nipple at the top of the four way crossover. It was observed that the well casing had not been worked on as required in the last inspection report, due to the very deteriorated condition of the above ground 8 and 5/8 inch casing. The valve on the tubing and to the flow line was closed. There was a domestic line piped from the tubing head with an open valve. The leak needs repaired immediately.

I spoke with Bob Barr Jr. , he told me he was not aware his employees had not completed the grading. Bob told me he is having equipment and employee problems. Bob told me he would address the final grading problem as soon as he could. Bob could not give me a exact time of completing the grading at this time.

1693586551 2012-08-21 Production Wells No Violations I received this complaint by e-mail from Paul Buser [ landowner ]. On my inspection of flow line route I found the ground has sunk 1 plus below grade. Also, there is an exposed section of 2 plastic line, 8 to 10 long, 200 south of the well head. The access road to well has been deeply rutted by company, not allowing landowner to access parts of his property. REMEDIAL WORK REQUIRED: bury flow line 24 below grade from wellhead to the separator at the tank battery and grade access road to the well. I spoke with Bob Barr Jr. about the relevant remedial work required in this complance notice.
