0317968780 2017-08-01 Production Wells No Violations I conducted an inspection on this well associated with complaint 5670. Landowner John Campagna owns the property that this well is located on and has requested the well to be plugged due to no production. The well was equipped with a plunger lift assembly and the annulus valve was open, but the flow line was closed. Production records for this well show no reportable production since 2012. The tank battery consisted of one 100 BBL production tank and a mechanical separator. Valves on the separator were closed. The tank battery was diked and needed updated signage to reflect the current ownership information. The tank battery was overgrown with brush. The lease access was stable. I contacted Phillip Dietrich, owner of this well about plans for this well. Mr. Dietrich indicated that he wanted to replace the plunger lift assembly on the well with a pumpjack, in order to be able to put the well back into production. I advised Mr. Dietrich to send a timeline outlining plans for putting the well back into production by 9/02/2017.
1010002620 2018-09-11 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1507143289 and complaint #5670. At the time of this inspection, he well was not in production and very overgrown with brush. Signage has not been updated with current owner information at the tank battery. No plugging permit has been obtained for this well. I called and left a message with well owner Philip Dietrich informing him of enforcement escalation to a Chiefs Order, involving this site. I spoke with landowner John Campagna regarding the enforcement escalation and observations at this well site, during this inspection.
1507143289 2017-10-04 Production Wells Violations Noted I conducted a follow-up inspection on this well from the previous inspection conducted on 8/1/2017. There has been no changes to the well site and the requested submission of plans by 9/2/2017 regarding this well from Mr. Dietrich, have not been submitted. The well was equipped with a plunger lift assembly and the annulus valve was open, but the flow line was closed. The tank battery consisted of one 100 BBL production tank and a mechanical separator. Valves on the separator were closed. The tank battery was diked and needed updated signage to reflect the current ownership information. The tank battery was overgrown with brush. The lease access was stable. The well owner operator shall also submit a written plugging schedule to inspector Shannon Mullins at 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685 /within 30 days. Per OAC 1501:9-11-05(B), and plug the well within 150 days. The well owner/operator will be notified if the plugging is approved or denied.

Signage has not been updated to reflect the current ownership information. The lease access was stable. I contacted Phil Dietrich, well owner about the status of the well. Mr. Dietrich indicated that he would be making application to plug the well within the near future.

The well was equipped with a plunger lift assembly and the annulus valve was open, but the 2 inch flow line was closed. The tank battery consisted of one 100 BBL production tank and a mechanical separator. Valves on the separator were closed. The tank battery location appeared to have been cleared of vegetation and a soil pile was left next to the 100 BBL tank. The dike has been removed. Signage still needs updated to reflect the current ownership information. No plugging permit has been issued for this well. I contacted well owner, Phil Dietrich and left a message about aquiring a plugging permit.
1533220725 2018-08-02 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1507143289 and complaint #5670. I was on site with John Fleming, inspector for DOGRM. At the time of this inspection, no plugging permit has been obtained for this well. The well was not in production at the time of this inspection. Signage has not been updated with current owner information. I contacted well owner, Phil Dietrich and left a message about aquiring a plugging permit.
